Client: My client is a 20-year-old lady named Skyler, accompanied by her 49-year-old dad, Mr. Brown
Chief Complaint: The father reports that the client feels like she is going crazy. She had severally tried to cut herself and threatened to kill herself.
History of the Presenting Illness
The patient reports that she is overwhelmed by what is going on in her life and feels she cannot take any more. She admits to having been in an emotionally abusive relationship with her boyfriend. The boyfriend was controlling and picked on everything she did. She always had to remain careful about what she says or do to avoid upsetting him to not yell on her. The boyfriend was not physical, though. Over time, she grew anxious and depressed. She quit going to work since she could not handle it. She would stay indoors, break down, and cry. She felt she needed help and admitted herself to a local hospital in May. However, during this moment, she was no suicidal. In the hospital, the physician put her on Trazodone, Paxil, and Ativan. She was not given a follow-up. After this, she moved out of the relationship, left the city, and came to live with her father with the hope of freeing herself and getting better.
Things here were no better. Her father lives with his mother and her two other half-siblings. He is a drunkard, with no job and living off his mother's money. The patient had to be responsible and take care of her siblings since the father was too drunk to care for them. She has been here with her father for 8 weeks now. She wants to go back to and live with her mother but feels guilty about leaving her siblings alone with her father. She needs a job, but she can't figure out how to get one on her own. She feels worthless and with no energy to continue living.
Allergies: She has no known drug allergy
Past Psychiatric History: She was recently diagnosed with anxiety and depression. She is currently on Trazodone, Paxil, and Ativan.
Past Medical history: She has no past medical history.
Substance Abuse: She takes marijuana and drinks alcohol occasionally. She does not use tobacco or any other illicit drug.
Her parents separated while she was 3 years old. The dad remarried and has 2 more children. The mother did not remarry and lived alone in a small city nearby. She has been visiting both parents to spend time with them. Her mother has no other child, but her. She works in a nearby grocery store. Her father usually attends family counselling at the facility and presents with complaints of feeling depressed abo...
