1 Complete Medical History
This interview is conducted on patient C, who negated the idea of providing his name for the interview.
The Patients Social Hx: Heavy smoker smoking 20 cigarettes/day over the past 14 years old (54 pack-years). Currently lives in single roomed house with his wife. The patient retired 5 years ago from a factory that dealt with the production of genetic food nutrients. The patient gave up on alcohol 5 years ago. In order to determine the patients complete medical history, the following interview was conducted;
Doctor:When did you first notice you had a shortness of breath during rest or exercise?
Patient: I have noted an intense shortness of breath over the past few months. In most cases, even when not engaging in any strenuous activities, I experience this problem.
Doctor:How often does this occur?
Patient: Well when engaged in activities the shortness of breath becomes intense. During rest, I may also experience this though not intense as when doing an activity.
Doctor: How long have you been experiencing a shortness of breath? Is this condition worsening?
Patient:I have been experiencing a shortness of breath for the past three months. The condition is however worsening since I have not noted any changes.
Doctor:How far can you walk, and if so, how many steps do you make before stopping as a result of shortness of breath?
Patient:Well I may not establish the exact number of steps I can make but I do not go far before experiencing this issue.
Doctor:How often and when do you mostly cough?
Patient:I have frequent coughs especially when conducting activities that are strenuous.
Doctor:How long have you experienced this coughs? Are they persistent?
