This week I counseled a patient who is a 66 years old Hispanic male. This patient has been admitted to Nursing Home due to Psychotic disorder, sleep disturbances, depression, and hypertension. A psychiatrist saw the patient following a report by the staff that the patient refuses care. According to the medical examination, the patient is obese. The patient does not engage in regular exercise. To counter the severe effects of sleep disturbance, the patient is currently on Trazodone 25 mg p.o QHS, Mirtazapine 7.5mg p.o QHS to treat depression, and Seroquel 25 mg p.oqhs for psychosis. No Gradual dose reduction (GDR) is recommended at this time because the patient is entirely symptomatic. As such, the patient is expected to continue with current medication. A nurse has been selected to follow up in 3 months.  
Using the DSM-5, explain and justify your diagnosis for this client.
The DSM-5 is an approach that introduces a dimensional approach to diagnosis and classification with current approaches. Using DSM-5, this patient can be diagnosed with Neurodevelopmental disorders, Bipolar disorders, and psychotic disorders related to the expressed symptoms. Neurodevelopmental disorders consist of a group of mental health conditions like those exhibited by the male Hispanic patient (American Psychiatric Association, 2015). A psychiatrist saw him after the staff reported him since he refuses to care. Additionally, the development of the patient's nervous system is tampered with by depressive moments. For a patient to get diagnosed with bipolar disorder, one should have experienced some episodes of hypomania. The patient should have elevated or irritable moods for at least a week. After the patient was examined, these symptoms were present.
From the patient's interview, I noticed that sleep disturbances, depression, and hypertension could be linked to developmental brain dysfunction and stress that might be related to past traumatic events. The patient also had a significant weight gain and increased appetite. This explains why the patient is obese. The patient also complained of fatigue and diminished ability to concentrate almost every day. Although the patient did not have any suicidal thoughts, he said that he had feelings of worthlessness almost every day. Besides, this patient can be diagnosed with bipolar disorders that interfere with his moods, energy, and the ability to function. The prevalence of psychotic illness is linked to severe mental health conditions that cause abnormal thinking and perceptions. The patient refuses care and depicts that he is suffering from a psychotic disorder that he has been treating presently (Boschloo et al., 2015). Therefore, these mental disorders diagnosed on the patient should be attended to by a psychiatrist and proper measures to enhance well-being.
