Culturally Sensitive Care
Culturally sensitive care is not only necessary but also extremely important in the modern healthcare environment characterized by patients from diverse ethnic, cultural, and linguistic backgrounds. Culturally sensitive care is the delivery of care characterized by appropriate knowledge, appreciation, and understanding of all cultural differences resulting in optimal health outcomes (Ragavan et al., 2020).
Culturally sensitive care is extremely valuable in healthcare. In an article by Diaz et al. (2017), regular use of Cultural Formulation Interview (CFI) was found to be helpful to physicians in identifying the unique preferences and needs of patients. As a nurse, I had encountered a situation when a patient considered treatment from a spiritual doctor. I believed the choice of treatment would put them at risk, but I sought to understand their view, listened to them, explained the proposed treatment, and answered questions they had. Ray (2016) pointed out that transcultural communicative spiritual-ethical caring is characterized by awareness and an understanding of the patient in the context of caring.
Theory and Organizational Framework of the Purnell Model
The Purnell Model is characterized by a circle with the outermost ring representing the global society, the second one depicting community, the third one family, and the inner one representing the individual (Purnell & Fenkl, 2019). Inside the circle lie 12 pie-shaped wedges representing cultural constructs and their corresponding concepts. The center of the circle is dark to depict unknown phenomena. A jagged line lies below the model to represent “the nonlinear concept of cultural consciousness” (Purnell & Fenkl, 2019, p. 8). Purnell and Fenkl (2019) pointed out that the 12 cultural constructs and their corresponding concepts provide the organizing framework. Each construct contains concepts that should be considered when examining patients in different settings. It is important to note that none of the domains stands alone; all of them are interconnected.
Purnell’s 12 Domains of Culture (Purnell & Fenkl, 2019)
1. Overview and Heritage
This includes concepts related to one’s current residence and the home country and the influence of the home country and the current home’s topography on health politics, education level, occupation, and economics. As an OR nurse, when assessing patients, taking into account their home country and current residence is essential given the diversity of patients we serve.
2. Communications
It covers concepts connected to the dominant language, dialects, and aspects such as intonations, tones, voice volume, and inflections (Purnell & Fenkl, 2019). Other concepts within this domain are nonverbal cues such as body language, use of touch, eye contact, facial expressions, and gestures. Asking about their name, assessing their language, and their response to touch form a key part of evaluating patients.
