Health History
Mr. M., a 70-year-old male, has been living at the assisted living facility where you work. He has no know allergies. He is a nonsmoker and does not use alcohol. Limited physical activity related to difficulty ambulating and unsteady gait. Medical history includes hypertension controlled with ACE inhibitors, hypercholesterolemia, status post appendectomy, and tibial fracture status postsurgical repair with no obvious signs of complications. Current medications include Lisinopril 20mg daily, Lipitor 40mg daily, Ambien 10mg PRN, Xanax 0.5 mg PRN, and ibuprofen 400mg PRN.
Case Scenario
Over the past 2 months, Mr. M. seems to be deteriorating quickly. He is having trouble recalling the names of his family members, remembering his room number, and even repeating what he has just read. He is becoming agitated and aggressive quickly. He appears to be afraid and fearful when he gets aggressive. He has been found wandering at night and will frequently become lost, needing help to get back to his room. Mr. M has become dependent with many ADLs, whereas a few months ago he was fully able to dress, bathe, and feed himself. The assisted living facility is concerned with his rapid decline and has decided to order testing.

He is also seen wandering outside of his room at night and is always in need of assistance to get back to his room. His medical history includes hypertension and hypercholesterolemia, which are managed using Lisinopril (20mg-1dose per day) and Lipitor (40mg- 1 dose per day). He has also had appendectomy and repair of a tibial fracture. He lives a sedentary lifestyle since he has difficulty ambulating. Additional medication that he thrives on includes Ibuprofen (400mg), Xanax (0.5mg), and Ambien (10mg). His Body Mass Index is 27.9, which indicates that he is overweight.
Primary and Secondary Diagnoses
Alzheimer's disease is a primary diagnosis that would be considered for Mr. M's case. Poor judgment, poor concentration, language changes, inability to reason effectively, and memory decline are symptoms of Alzheimer's disease (Liu et al., 2019). In addition, Mr. M. exhibits many risk factors for the disease, including physical inactivity, old age, hypertension, and overweight and obesity. Delirium, related to Urinary Tract Infection, is a secondary diagnosis that would be considered following Mr. M's case. The etiology is linked to a rise in the white blood cells count (19.2), a rise in the lymphocytes level (6700), positive urinalysis for a moderate amount of leukocytes, and cloudy urine.
Expected Abnormalities
In the process of nursing assessment, three significant abnormalities are likely to be seen. They include involving the relatives and primary caregivers in the assessment. Since Mr. M's mental status has deteriorated, he might not be able to give his account of what he feels and goes through.
