100.0 %Develop an evaluation plan. Describe the rationale for the methods used in collecting the outcome data, the ways in which the outcome measures evaluate the extent to which the project objectives were achieved, and how the outcomes will be measured and evaluated based on the evidence. Address validity, reliability, and applicability. Describe strategies to take if outcomes do not provide positive results and the implications for practice and future research.
Evaluation plan presents the method used in the measurement of the outcomes but does not align the project objectives and/or evidence. The rationale is missing. Modification strategies as well as the impact to future research may or may not be present with minimal details. Subject matter is absent, inappropriate, and/or irrelevant. Surface errors are pervasive enough that they impede communication of meaning. Inappropriate word choice and/or sentence construction are used.
Evaluation plan presents the methods used in the measurement of the outcomes according to the project objectives and/or evidence. The rationale is missing. The information presented is ambiguous. Modification strategies as well as the impact to future research may or may not be present with minimal details. A lack of comprehension is displayed, but there is an attempt to apply information. There is weak, marginal coverage of subject matter with large gaps in presentation. Frequent and repetitive mechanical errors distract the reader. Inconsistencies in language choice (register), sentence structure, and/or word choice are present.
Evaluation plan presents the rationale for and the methods used in the measurement of the outcomes according to the project objectives and evidence. The information presented may lack cohesiveness and details. Possible project modifications when outcomes do not provide expected results are listed. The implications for practice and future research are broadly described. Comprehension of the material is exhibited and there is clearly an attempt to integrate and apply information. All subject matter is covered in minimal quantity and quality. Some mechanical errors or typos are present, but are not overly distracting to the reader. Correct sentence structure and audience-appropriate language are used.
Evaluation plan presents the rationale for and the methods used in the measurement of the outcomes according to the project objectives and evidence. Validity, reliability, and applicability are briefly addressed. Strategies for project modification when outcomes do not provide expected results are outlined. Implications for practice and future research are broadly described. Integrative and accurate comprehension is demonstrated and information is applied as appropriate. There is comprehensive coverage of subject matter. Prose is largely free of mechanical errors, although a few may be present. A variety of sentence structures and effective figures of speech are used.
Evaluation plan presents the rationale and the methods used in the measurement of the outcomes according to the project objectives and evidence. The information presented is aligned, comprehensive, and addresses validity, reliability, and applicability. The plan formulates clear and precise strategies for project modification when outcomes do not provide expected results. An informed position on the implications for practice and future research is developed and explained. Clarity and specificity of comprehension is demonstrated and all relevant information is synthesized. Coverage extends beyond what is needed to support subject matter. Evidence-Based Proposal: Evaluation Of Process Institutional Affiliation Name Evidence-Based Proposal: Evaluation Of Process Introduction The effects and the negative impact of sepsis is a menace that has devoured and claimed lives of many patients all over the world. In the United States of America for instance, sepsis has affected three-quarter of a million patients in just the past one decade where the individuals have a mortality rate of 28% (Dellinger, Levy, Rhodes, Annane, Gerlach, Opal, & Osborn, 2013). A diagnosis of sepsis usually has a mortality rate of 30 to 50%, account for 20% of admissions to an ICU and will cost an average of $22,000 per case. These sepsis cases cost $17 billion annually just in the United States (Castellucci, 2017). The surviving sepsis campaign champions awareness increase, mortality reduction, and improved patient outcomes. The evaluation will purpose on reducing the effective medical care facilities and hospitals that the sepsis patients are given. This will be done by implementing a sepsis time clock system that will help the nurses to use to help the sepsis patients get the much-needed medication on time and effectively. The sepsis time clock will be implemented in two forms, one on a paper and the other one as a real timer in the rooms where the patients are. The timer clock and the paper clocks will work in bundle elements through three bundles of time. The Stetler model of change will be used to help the institutions to be very effective and run smoothly for the institutions and hospitals that are going to adopt the proposed sepsis time clock. Every stakeholder will benefit in the evaluation as all the parameters of interest like capital and time are carefully considered as the stakeholder play an important role in the implementation and developing of the evaluation plan. The methods used for data collection methods will be an integral part of analyzing the outcomes of the evaluation plan of sepsis clock time. One of the success parameters that will highly associate with the sepsis time clock is the number of deaths that it causes directly or indirectly. A study conducted in 2017 showed that sepsis was responsible for 15% of more than 2.9 million of the death in 409 hospitals across the United States between 2011 and 2014 (Lee, Ramar, Park, Gajic, Li, & Kashyap, 2013). This sepsis time clock aims at reducing the number of deaths that this disease condition is causing in the United States of America. The data collected will involve the general and specific data from the institutions that are going to implement the sepsis time clock evaluation plan. The six phases of the Stetler change model will careful...
