History of the Patient
The patient’s name is KT. She was borne on 12th December, 1978 in Seoul, South Korea and is aged 39 years. She speaks English and Chinese. The patient lives with her mother and 2 little boys aged 10 and 8 years in 1234 Willow St. San Jose, CA 95161.She is a Chinese immigrant who came to America in June 1998 with her parents and younger brother. She divorced a year ago and is currently dating. She holds an associate degree in Business Administration and works fulltime at one of the high tech firms in San Jose, California as a clerical officer and has health insurance.The objective of the clinical appointment was to assess KT’s health status after she gained 16 pounds in 8 months.
Diagnosis/Analysis of the Patient
Based on the patient’s history and biodata, her weight gain can be linked to her recent divorce coupled with a stressful fulltime job at a tech company. Given that she just gained 16lb in 8 months, yet she divorced just 12 months ago, a link can be established between the two incidents.Given that she is a middle aged Chinese woman, she might have been hard hit by the psychosocial implications of the divorce. This is evident from the fact that she is already dating. Still, she lives with two boys aged 10 and 8 years respectively. Research indicates that boys who fall within this age bracket tend to eat a lot, and hence they might have as well influenced her eating behavior. During the first visit, the client’s psychosocial status will be examined through psychotherapy.
KT is scheduled to attend the next follow up in two weeks’ time. This follow up visit is crucial as it will present an opportunity for her psychosocial progress to be assessed since it is the likely source of the rapid weight gain(Long-term, 2017). During the follow-up, the patient will be monitored for comorbid conditions. Monitoring will be done to establish the patient’s response to effective management of diet (Brunault et al., 2015). Thirdly, the follow-up will evaluate the patient’s weight loss response and identify possible treatment complications and possible weight re-gain risk factors for effective management.
The next Return To Clinic (RTC) follow-up is specifically important for overweight patients considering that theycould encounter psychosocial or treatment challenges. This visit will largely focus on psychosocial counseling, coping evaluation and progress monitoring. However, during other subsequent follow-ups, KT will be psychologically counseled to accept her new physical image and how to maintain her new look.Psychosocial counseling is critical since it has a profound effect on the patient’s diet, long term health and wellbeing (Brunault et al., 2015). The follow-up will last between 6-12 months even though it could be shorter depending on how fast the patient will respond to treatment.
