By Day 1 of this week, you will be assigned to a specific case study for this Case Study Assignment. Please see the “Course Announcements” section of the classroom for your assignment from your Instructor.
Also, your Case Study Assignment should be in the Episodic/Focused SOAP Note format rather than the traditional narrative style format. Refer to Chapter 2 of the Sullivan text and the Episodic/Focused SOAP Template in the Week 5 Learning Resources for guidance. Remember that all Episodic/Focused SOAP notes have specific data included in every patient case.
With regard to the case study you were assigned:
Review this week's Learning Resources, and consider the insights they provide about the case study.
Consider what history would be necessary to collect from the patient in the case study you were assigned.
Consider what physical exams and diagnostic tests would be appropriate to gather more information about the patient's condition. How would the results be used to make a diagnosis?
Identify at least five possible conditions that may be considered in a differential diagnosis for the patient.
The Case Study Assignment
Use the Episodic/Focused SOAP Template and create an episodic/focused note about the patient in the case study to which you were assigned using the episodic/focused note template provided in the Week 5 resources. Provide evidence from the literature to support diagnostic tests that would be appropriate for each case. List five different possible conditions for the patient's differential diagnosis, and justify why you selected each.It is highly recommended that you access and view the resources included with the course text, Seidel's Guide to Physical Examination. Focus on the videos and animations in Chapters 7 and 23 that relate to the assessment of cognition and the neurologic system. In this case, it is important to conduct five diagnoses and diagnostic testing on the patient.

HPI: The patient is complaining of suffering from intermittent headaches. The patient is a 20-year-old man. He had experienced headaches for the past three weeks. The headaches happen diffusely but the intensity is felt around the nose, cheekbones, jaw, and oboe the eyes.
Location: Jaw, cheekbones, above the eyes, and through the nose.
Onset: The patient has had intermittent headaches for the past three weeks.
Character: The patient is dull but the symptoms intensify as aching increases.
Signs and Symptoms: The patient has been experiencing a lot of pressure and stress from the job.
Timing: The headache occurs different times in a day
Relieving factors: The patient had taken some Ibuprofen to relieve pain several times.
Sternness: 6 out of 10 at its greatest intensity.
Current Medication: Ibuprofen to relieve pain
Allergies: The patient is not allergic to any drug
Past Medical History: The immunization of the patient is up to date. The last tetanus shot of the patient was conducted in July 2025. There is no significant medication history.
Soc Hx: The patient is a college student who is pursuing sports management. The patients also work as a full-time employee. He likes watching and playing sports during his free time. He lives with his parents. The patient does not abuse any drugs. The patient also wears a seat belt anytime he is traveling in a motor vehicle. The patient also owns a cell phone but he hardly uses it when driving. At his parent’s house, there are smoke detectors.
Family History: The mother of the patient is alive but she is suffering from diabetes. The father of the patient is also alive and he has high blood pressure. However, the younger sister does not have any medical issues. The grandparents of the patients are dead due to natural causes.
ROS
General: No body weakness, fatigue, weight loss, or fever.
HEENT: The patient has no visual loss or blurred vision. He has no hearing loss, sore throat, runny nose, or sneezing.
Skin: No itching
Cardiovascular: The patient does not have any chest pains, chest discomforts, or chest pressure. There are no cases of edema.
