Summary of the Problem
The increasing occurrence of falls among patients in healthcare facilities is a cause for alarm to all the stakeholders in the healthcare industry. Falls in hospitals can lead to adverse repercussions on the patient’s well-being due to the sustained injuries, which further prolongs their healing process. According to the Agency for Healthcare Research and Quality estimates, close to a million patients fall in hospitals and other healthcare facilities in the United States every year. The risk factors for falling associated with the patient’s health include their history of falls, side effects from psychotropic and hypnotic medication, and activity intolerance. Poor management and structural organization of the health care facility may also increase the risk factor of falls through insufficient staffing of nurses, poor communication between and among the health care providers, and poor maintenance of the infrastructure. The rising cost of health care in the U.S. and around the world calls for an effective nursing intervention to prevent falls in the healthcare facilities. The professional practice facility deals with veteran patients aged 25 to 98 years, a bed capacity for 28, and a medical-surgical unit for the adults. A middle range theory referred to as the Neuman’s Systems Model (NSM) served to solve the highly challenging issue of falls and facilitated the change of attitude and approach among the nurses towards fall prevention. The model’s significant impact was in the education of nurses on identifying patients at high risk of falling as inaccuracy in previous identification techniques compromised the prevention measures taken by the nurses in hospitals.
Borrowed Theory
The proposed borrowed theory befitting the facility is the PRECEDE-PROCEED model as it demands an inclusive approach in achieving the desired positive outcomes of reducing the influence of the risk factors and thus preventing and or reducing falls (Porter, 2015). The PRECEDE-PROCEED model, as the name suggests, is compartmentalized into two simultaneous components, PRECEDE and PROCEED, each serving an integral part in developing the intervention plan for fall prevention (Green, 2005). PRECEDE (Predisposing, Reinforcing and Enabling Constructs in Educational Diagnosis and Evaluation), mainly deals with the identification of the problem and the factors propagating the risk of falls, upon which countering measures develop (Green, 2005). PROCEED (Policy, Regulatory, Organizational Constructs in Educational and Environmental Development), on the other hand, focuses on ensuring that the proposed prevention measures from the PRECEDE segment of the model are developed, implemented, and evaluated accordingly (Porter, 2015). The model’s two major components have four stages, each serving a unique purpose in the development and implementation of the fall intervention program.
