Vision
It has been established that Miller’s family and other families in the community are underserved when seeking care. The major problems they are facing include substance abuse, chronic illness, and minimum insurance. As a result, family members have little or no access to community care. Further, it has been established that community-based solutions fronted by community members are crucial to helping such families. The purpose of this community-based initiative, therefore, is to help Miller’s family and other similar families within the community to have minimum essential coverage. Therefore, the vision of this plan is to ensure that these families have insurance plans that meet the Affordable Care Act Requirements to enhance their access to care.
Underlying Health Complications
In Miller’s family, both parents have a history of heavy substance abuse in their late teens and early adulthood. Both parents are obese smokers with the mother diabetic with hypertension while the father has hypertension and liver complications. Therefore, parents require therapy that will help them overcome drug issues. Further, the family needs help in actualizing practical measures that will help in weight and diabetes management. The implication, therefore, is that the mother needs access to insulin and other diabetic-related medical solutions. The father also needs his liver and blood pressure to be checked. The 14 years old son, who is still dependent on law, requires health care to manage his asthma condition. The 18 years old daughter is showing signs of drug dependence; she may have had two abortions. Because of the family’s situation, the members are not able to access adequate care for their well-being and the overall well-being of the community. For the teenage son, there is a need to determine if he is receiving adequate care, and if the family is providing a conducive environment for his growth and development because he is a minor by law.
Approach
Problems faced by Miller’s family and other similar families consist of various intertwined social, political, and economic constraints. Thus, there is need to gather extensive information on such families within the community. In association with community health workers, families undergoing similar problems such as Miller’s family will be identified, and their needs highlighted. In the process, the causes of the predicaments are determined and potential intervention points that promise broad reach and impact are considered.
