He has difficulty maintaining diet restrictions and managing his polypharmacy. He has 4+ pitting edema, moist crackles throughout lung fields, and labored breathing. He has no family other than his wife, who verbalizes sadness over his declining health and over her inability to get out of the house. She is overwhelmed with the stack of medical bills, as Mr. P always took care of the financial issues. Mr. P is despondent and asks why God has not taken him.

Cardiomyopathy is a medical condition where the muscles of the heart fail to perform to the normal rates that they are supposed to. The disease impacts the cardiac muscles to an extent that they can no longer be as efficient as before, hence impacting the normal functions of the heart, such as pumping blood to various parts of the body. Mr. P is suffering from congestive heart failure, but at the same time, seems to have a number of other things happening with him.
Approach to Care
Even before getting to the medical aspect of it, Mr. P needs to be counseled intensively and extensively with the goal of helping him look at life positively. Counseling will also help him have a better perception of himself, and hence help generate some joy and happiness in him. The first step towards treatment and recovery or disease management is usually psychological stability (MacRae, 2010). Mental health is just as important as physical health. It entails not only the dietary observation but also carrying out basic, simple physical exercises after the drugs have stabilized the cardiac muscles. These exercises should be progressive, so that the muscles are strengthened gradually.
Treatment Plan
Once this is handled, the second step would be a combination of both nutritional therapy and medical prescriptions. The goal is to ensure that the patient’s heart is strong enough to carry on with its functions. Some of the medications that can be used include ACE Inhibitors and Beta Blockers (Grewal, 2012).Nutritional therapy would also be important, since Mr. P needs a diet that will help in strengthening the heart. The physician in charge of Mr. P’s treatment will have to do a thorough background check such as the family history of Mr. and any other valuable information that will give him clues on what kind of medication to do it.
