Trouble in the Truss Construction Shop Two weeks ago, during a QA truss load test, the truss being tested fragmented along a horizontal axis, causing a large piece of the truss to break part and fall on a hoist operator supporting the test. The hoist operator sustained head injuries and remains in an induced coma in a local hospital. This accident sent shock waves through the Truss Construction Department because the company has heavily invested in a new engineering and manufacturing process to produce a cost-effective truss that has been touted to be on the “cutting edge” of construction technology, especially for low cost housing in overseas markets. A report by the Safety Officer, QA manager, and engineer verified that the test being conducted pushed the load testing slightly beyond the high threshold of acceptable load-bearing, though the extra load was not expected to cause the truss to fail. In fact, the trusses were advertised to meet “commercial-high” load requirements. Employees in the engineering shop have been asking if the manufacturing or engineering process is flawed and if the trusses being produced could fail under load. Company memos have focused on production and more testing at lower thresholds, and members of management are encouraging employees to continue the current production schedule to meet orders for the trusses. The Sales department is highly concerned that if there is any delay in shipping, customers will pull their orders, which would have a disastrous result on revenues. Faruch Habib, a production line worker, leaked the details of the accident and test thresholds to the press. Two weeks later, he was terminated for documented poor performance, according to managers. The company Public Relations department has issued a general statement that the company has taken all action to ensure that this type of workplace accident would not be repeated.

Introduction
Construction workers are often exposed to various safety and health risks in the course of their work. According to the United States Department of Labor (2018), employees in the construction industry are faced with hazards categorized as the "fatal four" which include falls, being hit by a falling object, electrocution, and getting caught in between two objects. A focus on construction work involving the installation of roof truss shows that there are significant hazards that workers have to contend with. It follows that trusses are both flexible and awkward until they are fitted onto rooftops; thus, there may be injuries to workers or nearby people if anything is missed (Blanchina, 2009). The management and workers must prioritize and follow safety procedures in such work environments, as this will go a long way in preventing hazards or minimizing their impact when they occur. This paper is based on a case in a truss construction shop where an employee sustained a head injury after being hit by a fragmented truss. Here is an explanation of the problem, followed by an analysis of the information, consideration of different views, and conclusions and recommendations.
Explanation of the Issue
The truss construction company under focus in this paper has managed to appeal to customers by developing a new cost-effective truss. Accordingly, some customers have pre-ordered the product as they are convinced that it's a better option compared to what is available in the market. However, following the accident where a loaded truss fragmented and injured a worker at the truss shop, there has been negative publicity to the company and customers are beginning to think otherwise. The accident was severe that it sent the operator into a coma and had to be admitted to a nearby hospital. This dominated public discussions, which is not suitable for the company's image. Moreover, the continued focus on truss production in a bid to meet customer demands and generate more revenue; while there is a bad incident only projects a bad image of the company.
Analysis of the Information
After the incident involving a worker at the truss company, the security and value bosses, and the engineer on site investigated the accident to ascertain its cause. Their findings indicated that the load testing was at fault because it exceeded the peak of load-bearing, while the expectation that the truss would hold extra load did not help the situation. Accordingly, the truss should have been tested at a lower threshold to be sure of its structural integrity. Additionally, the company is faulted for wrongly advertising that its trusses met "commercial-high" freight recommendations and failing to listen to employees who questioned the credibility of the production processes. Failure to disclose the truss load issues with customers further exposes the company in a bad light, while a lack of ethical values is evidenced by the company's attempt to shield the problem internally. Another issue regards retaliation, as noted by the company's action of firing a concerned employee who shared the incidents with the press. Interestingly, the company claims that the ...
