My patient that I chose is;
85 years old lady living with her husband 97 y/o who has a dementia but without behavioral disturbance. She is heathy overall and takes care of her husband as well.
This lady visited Dr.’s office with complaint of sleeping disturbance due to her husband.
She states that her husband is forgetful and watches and follows her all the time. Her husband is able to walk and talk and follows activities of daily living normally but sometimes, he does not understand her words and acts differently, and her husband does not let her go out by herself when she wants to do something on her own for grocery shopping. She states that she gets upset and nervous about her husband’s behavior. She is worrying about her husband may make an accident. So She has to be careful about what he does which makes her busy and then she cannot take a rest.
This bothers for her sleep at night as well which makes her wake up and worry about her husband more. She states that she is in despair and suffering from her husband's dementia.
She is taking medications for hypertension, Diabetic type ll, hyperlipidemia, hypothyroidism, and depression disorder.
Dr. ordered Mirtazapine 15 mg to aid her sleep on top of her esitalopram 10 mg once a day and advised her to make an appointment with Dr. for her husband.

The eighty-five years old lady lives with her ninety-seven-year-old husband. The lady is determined to be healthy, and she takes care of the husband. However, the client complains about sleeping disturbances, and this has been attributed to challenges caused by her partner, who has dementia. She reports that sometimes her husband acts differently, and sometimes he does not let her go out by herself, particularly when the patient needs to do shopping. Besides, the patient reports that she is nervous about her husband’s behaviors and also worries that he might cause an accident. Further, the patient is in despair and which is caused by the partner’s condition. Often, despair is common among both women and men; however, women are more likely to report the condition.
Analysis and response
Most often, individuals with despair tend to experience a sense of defeat and lack of hope. Besides, people with despair are likely to become helpless and powerless. The patient has reported that she feels hopeless. It is critical noting that despair would be a sign of depression. In most cases, despair would persist after depression treatment, and patients would be needed to receive pharmacological psychotherapy interventions (Cowling, p.288). Because the patient is a Christian, it is critical to examine whether she has spiritual distress, and this would be associated with loneliness, restlessness, sleep disturbances and feeling guilt.
