What are the minimum eligibility requirements to apply for this grant? The selected waivers have to meet specific requirements before awarding the grant. All applicants must have a prior qualification in the New York State Grants Gate without any links with profit-making organizations. The contractors should have a minimum of three years' experience in Nursing Home Transition and Diversion (NHTD), giving satisfactory services to the seniors and people living with disabilities.
