In your initial comment you should focus on what you have ascertained to be the most important issues associated with the effective delivery of human services. Some of these issues include client progress, self-assessment, and worker effectiveness. What worker competencies are necessary, and what constitutes quality case management?

The most important issues in human services delivery include group work, continuous learning, and client advocacy competencies. Group work competency involves a professional’s ability to work as a team aiming at creating good coordination and relationships to achieve desired objectives. Continuous learning competency demonstrates an ability to utilize reflective practice depending on the available evidence to evaluate and improve in services delivery as in interprofessional groups (Langins & Borermans, 2015). Client advocacy competency refers to the ability to advocate the client needs such as patient medical delivery to promote the best care and strengthening them to become full and active participants towards their healthcare services (Halley, Koop, & Austin, 1998).
Case management constitutes complex health practices that are important in social care interventions for people with serious health conditions such as rehabilitation counseling, assertive outreach, chronic care. and post-acute hospital problems. Community-based case management is more preferred because it is patient-centered and meets their needs (Lukersmith, Millington & Salvador, 2016).
