Thank you for sharing your educative post about Migraines and tension-type headaches in regards to their presentation, pathophysiology, assessment, diagnosis, and treatment. I agree with the information you have provided about the conditions. It is true that non-steroidal anti-inflammatory drugs (NSAIDs) are the first line of treatment for migraines. Physicians often choose ibuprofen as the first-line treatment due to its availability and tolerability. For mild to moderate pain, ibuprofen 400mg q4-6hr PRN should be taken (Ibuprofen, 2019). The prescription includes one tablet at the onset of headache and take one tablet in 2 hours if needed but not to exceed 200mg/24 hours.
Although the first-line treatment for both conditions may be the same, I would prefer Amitriptyline for chronic tension headaches because it influences the body’s use of serotonin and norepinephrine, reducing depression and other types of chronic pain (Palacios-Ceña et al., 2019). In addition, prescription for adults includes 50mg, one to two hours before bedtime, to reduce grogginess on awakening (Palacios-Ceña et al., 2019).
