PICOT Question and Nursing Intervention on Poor Hand-Off Reports

Coursework Instructions:

In a paper of 500-750 words, clearly identify the clinical problem and how it can result in a positive patient outcome.
Step 1: Create PICOT question; A PICOT question is presented and provides a clear framework for the capstone project change proposal. Your PICOT question should clearly outline all of these elements: patient, intervention, comparison, outcome and time.
A PICOT question is presented and provides a clear framework for the capstone project change proposal
HERE IS MY PICOT QUESTION:
For adult patients in an acute care setting (P), does the implementation of bedside report during patient transfer of care (I) for 10 weeks (T) improve the patient communication and patient safety (O) as compared to hand-off report performed away from the bedside (C)?
Step 2: PICOT Problem: Identify the PICOT problem, what clinical problems or issues may arise from clinical care? The PICOT problem as it relates to evidence-based solution, nursing intervention, patent care, health care agency, and nursing practice is thoroughly described.
The PICOT problem as it relates to evidence-based solution, nursing intervention, patent care, health care agency, and nursing practice is described. Some minor detail is needed for clarity.
Step 3: Describe nursing intervention: A nursing intervention used to address the problem. Compare the nursing intervention to a patient population not currently receiving the nursing intervention, and timeframe needed to implement the change process.
A nursing intervention used to address the problem is clearly presented. A comparison of the nursing intervention to a patient population not currently receiving the nursing intervention, and timeframe needed to implement the change process, are thoroughly discussed.
Step 4: Summarize Clinical Problem and Patient Outcome: The clinical problem and how it can result in a positive patient outcome.
The clinical problem and how it can result in a positive patient outcome are thoroughly discussed. Strong rationale is offered for support.

Step 2: PICOT Problem:
Medication errors are regarded as preventable events in the medical field. Hand-off issues like transfer or shift change have been identified as primary contributing factors to hospital medication errors. This results in poor patient communication and safety. While most health practitioners believe that hand-off reports are essential since they prevent errors of omission, where they are poorly written can result in the deletion of important information. This leads to delays in the delivery of care among patients, miscommunication among patients, confusion, and extended stays at the hospital. According to Dorvil (2018), poor hand-off reports omit vital information like code status and medications. These reports also need better communication, illegible documentation, and face-to-face interaction.
According to Rolling, Pauley & Hoyt (2015), incomplete medical transfer jeopardizes patient safety, especially when they are being discharged from hospitals. One of the primary reasons for the failure of hand-off reports is incomplete patient information and the inability of other medical practitioners to receive transfer and discharge information. In acute care, these issues put the patient's health at risk considering that communication is also poor. Patients in acute care require adequate medical attention, and anything that limits this puts their safety and lives at great risk. Because of these issues, medical practitioners must look for an alternative like bedside reports and ensure patients in acute care receive quality health services.
