According to Dorvil (2018), poor hand-off reports omit vital information like code status and medications. These reports also need better communication, illegible documentation, and face-to-face interaction.
According to Rolling, Pauley & Hoyt (2015), incomplete medical transfer jeopardizes patient safety, especially when they are being discharged from hospitals. One of the primary reasons for the failure of hand-off reports is incomplete patient information and the inability of other medical practitioners to receive transfer and discharge information.
