In the article, Ziaeian and Fonarow (2016) discuss the heart failure (HF) problem, which continues to burden healthcare through hospitalization and readmission. The authors confirm that about 5.7 million individuals have been diagnosed with HF. Surprisingly, the number of individuals with chronic conditions is expected to exceed 8 million by 2030. Between 2008 and 2010, 67.4% of patients discharged were readmitted, while 35.8% died one year after discharge (Ziaeian & Fonarow, 2016).
