**Nurse Professional Standards Board**
Addressing the Four Dimensions (Nine Criteria) of Nursing Practice
The following guidance is provided to optimize your success with this process. Although daunting at times, your thoughtful responses to each of the Nine Criteria within the Four Dimensions are critical to the outcome of your grade. The NPSB has an opportunity to qualify you at one of three grades: Nurse I, Nurse II, or Nurse III. Within Nurse I, there are also three levels—Level 1, Level 2, and Level 3. When considering your responses, be advised of the following qualification standards for each grade:
Nurse I: Associate Degree/Diploma required. In each of the Four Dimensions, the Board is looking for INDIVIDUAL growth and development in clinical practice to increasing levels of self direction.
Nurse II: Bachelor’s Degree in Nursing required. (Associate in Nursing/Diploma with Bachelor’s in health-related field may be considered). In each of the Four Dimensions, the Board will expect UNIT level contributions and leadership.
Nurse III: Master’s Degree in Nursing required. (BSN with Master’s in health-related field may be considered). In each of the Four Dimensions, SUSTAINED LEADERSHIP resulting in program contributions and outcomes BEYOND THE UNIT/PRACTICE SETTING. That is, involvement at the ORGANIZATIONAL LEVEL is expected.
NOTE: Both educational requirements and meeting the Four Dimension standards for that particular grade are required to be recommended for that grade.
The Four Dimensions of Nursing Practice are as follows: Nursing Practice (Practice, Ethics, Resource Utilization); Professional Role-Leadership (Education/Career Development, Performance); Collaboration (Collegiality, Collaboration) and Scientific Inquiry (Quality of Care, Research). NOTE: Leadership is not one of the dimensions, but leadership principles are expected to be woven throughout the established categories.
Each of the Nine Criteria should be addressed by you citing examples (at least one) that are measurable and support the qualifications standards. One example may suffice to support all criteria under a single Dimension, but all criteria must be met…NOT just the Dimension. As a template for your response, the following guidance is offered:
**What is the issue/problem? – ISSUE/PROBLEM
What was done (intervention)? - ACTION
For Whom? - POPULATION
What difference did it make? – OUTCOME (Measureable)
When; Did it last? – SUSTAINABILITY
Please see the following table for additional guidance:

Four Dimensions of Nursing
I. NURSING PRACTICE DIMENSION: PRACTICE
1 Practice
Issue/Problem: Dealing with older adults with dementia presenting an altered cognitive process and are vulnerable to falling.
Action:
* Risk factor assessment for the patient falls. I initiated a review to indicate the likelihood of patients falling.
* Ensure that care is performed and documented
* Training staff on new fall prevention strategies. I organized training sessions to enable new and old staff members to prevent falls
* I enhanced regular monitoring by assigning each staff one to two hours rotation at the client's bedside.
* Initiated adjustment to address poor lighting and slippery floors
* Required nurses to keep the beds in low positions
Population: Elderly adults with dementia and presenting altered cognitive processes.
Outcome(s):
* Created a culture of responding to the elderly adult's inpatients within the shortest time possible
* Nurses could identify patients at a higher risk of falls
* Patient falls reduced by 60%
* Patients and family members discovered ways of avoiding falls
Sustainability: The intervention has been working well since its implementation.
2 Resource Utilization
Issue/Problem:
* The danger of overcrowding
* Instances of non-compliant with treatment regimen for patients suffering from erectile dysfunctions (ED). The clients would discontinue the treatment process or fail to adhere to the prescribed treatment plan, which increased treatment costs
* Instances skin ulcer posing threatening the lives of patients increasing treatment costs and hospital stays
Action:
* Required a bed occupancy rate of between 85 and 90%
* Trained nurses on how to engage with the non-compliant clients one-on-one to understand why they failed to follow the prescribed plan.
* Required the implementation of the odd (left side) and even (right side turn) hour rotation schedule.
Population:
* Patients who are non-compliant with the treatment regimen
* The nurses, therapists.
* Male clients' on psychotropic drugs, for example, Selective serotonin reuptake inhibitors (SSRIs).
Outcome(s):
* Bed occupancy rate reduced from 95% to 87% within two months; hence the hospital did not turn away patients.
* Nurses reported a 50% reduction in non-compliance
* Stigma for ED patients reduced significantly
* Teams were convened, and improvement strategies were undertaken based on the skin ulcer data provided
* Reduction in facility's overall falls and skin ulcer by 60%
Sustainability:
* The intervention worked for the entire period the ED patients were at the facility.
* The intervention was adequate for the entire time elderly patients and the dementia population were in the facility
3 Ethics
Issue/Problem:
* The need to offer culturally competent care. Instances for discrimination against African Americans and LGTBQ individuals were reported.
* Delays in admission because of limited staff members
* Increasing instances of employee gossiping of others competence leading to staff splitting behavior, which adversely affected ...
