Review of Case Study #3: Drooping of Face
A 33-year-old female comes to your clinic alarmed about sudden "drooping" on the right side of the face that began this morning. She complains of excessive tearing and drooling on her right side as well.
Subjective Data
Chief Complaint (CC): Right-sided facial droop
History of Present Illness (HPI): The patient had a right-sided facial droop since this morning, with excessive tearing in the right eye and drooling out of the right side of her mouth.
Medications: None
Allergies: None
Past Medical History (PMH): None
Past Surgical History (PSH): None
Sexual/Reproductive History: None 
Significant Family History: None
Additional questions for the patient
Do you have any history of stroke in your family? Are you having any pain? Is the pain persistent? Any tingling or loss of sensation anywhere? Are you having any issues with your vision? Have you been ill recently? What is your past medical history? Surgeries? Medications?
Additional Physical Examinations
I would perform a Cincinnati Stroke Scale and Glasgow Coma Scale (GCS) to examine the probability of mentation and stroke. I would inspect her head, face, and neck, neurovascular assessment in all 4 extremities thoroughly. I would also perform a swallow evaluation and assess her facial cranial nerve function by having her raise her eyebrows, squeeze her eyes shut, smile, puff out her cheeks, and purse her lips and blow out. This is expected to show more of a Bell ’s palsy.
Additional Diagnostic Testing
In case this was an outpatient setting, I would call 911 for transport to an emergency department for further evaluation. I would also contend upon a CT of the head to rule out an acute stroke. Consequently, I would contemplate extra electrophysiological testing on the cranial facial nerve if the acute stroke was ruled out to examine if the facial nerve is the culprit (Liebeskind, 2016).
