The community health project includes the community care plan and a project summary.  The student will conduct a windshield survey of a selected community and research local government websites to confirm data about the selected community and choose a specific social determinant that pertains to that community. The student will utilize the Future of Nursing: Campaign for Action Equity Toolkit (Campaign for Action, 2020) to create a plan of care including assessment, diagnosis, plan, implementation, and will provide an evaluation the specified social determinant in the community.

This assignment will be written as a word document per APA format. Please review APA format. This paper should be minimum of 6-8 pages and include a title page and reference page. 

The summary will include a minimum of three- five evidence-based references regarding care given for the social determinant by multiple disciplines. Also, the student will summarize three levels of prevention regarding the social determinant.

Dickinson, Texas, is a diverse city when conducting a windshield survey. Its high school graduation rate is 86.4%, while 24.1% of the citizens aged 25 and over have bachelor's degrees or higher. Besides, the median income is $77,311, and 79.1% of residents own homes. Predominantly Protestant, the city houses six churches and a Catholic school. Several industries in the area have employment opportunities in management, construction, and sales. The key social determinant in this community is transportation, as no public transportation is available, and the average commute time is 28.4 minutes. This determinant is critical in access to healthcare, and with public transportation, people may be able to get to necessary medical appointments and services. Hence it can significantly impact a population’s health.

According to the World Health Organization (2023), the social determinants of health (SDH) are “the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life.” SDH includes economic stability, education, food insecurity, health care, housing, social environment, and transportation.
Evidence-based practice shows that healthcare providers must consider the patient’s SDH when delivering quality care. For example, providers should assess patients’ access to transportation, housing, healthy food, and education. Healthcare providers must develop interventions to improve access to these SDH (Andermann, 2016). A nurse should ensure that the patient can access quality care and provide information and referrals to help address SDHs.
Healthcare providers should consider how SDH can impact access to health care. The research from the Archives of Public Health (Raghupathi & Raghupathi, 2020) revealed an association between social determinants and access to healthcare. Lower educational attainment, income, and employment levels were linked to reduced access to services, with lack of health insurance, poor health literacy, and geographic and travel barriers being some of the primary limiting factors.
As well the SDH can have an impact on health disparities. For instance, communities with limited resources may have difficulty accessing preventative healthcare services (Andermann, 2016). Individuals from racial and ethnic minority groups are more likely to experience health disparities due to SDH.
Health Impacts of Social determinant
Most residents of Dickinson either walk or drive to their destinations, with no public transportation available. As such, this can impact the population’s health, particularly for those with limited mobility or resources.
The lack of public transportation in Dickinson can primarily limit access to resources and social services, such as health care, educational opportunities, and recreational areas. Residents lacking access to wellness facilities, schools, and recreational areas may suffer from poorer health outcomes. Recreation activities can potentially improve physical, social, and mental well-being.
