Develop a preliminary evaluation plan for a program you are familiar with in your current work environment. The component of each SLP are outlined below:

The program will be geared towards Health specifically on pre- diabetes mellitus and signes of hypertension leading to chronic renal disease.

In a 2-page paper, develop a communication plan. How will you share the results of your program evaluation? Discuss who will receive the report,(Student Health Services and Student Affairs Services) etc. and why. Briefly, describe the role of a good communication plan in the evaluation process and how it can impact student success.
SLP Assignment Expectations
Your work should be written with the following points in mind.
• This paper should be at least 2 pages, not counting the title page and reference page.
• A clear introduction that orients a reader to the essay main content, and the main points discussed should be provided.
• A well-developed, well-balanced essay body that develops each point in its own paragraph should be included.
• A concise conclusion that summarizes the whole essay should also be included.
• Include at least three references. Quoted material should not exceed 10% of the total paper (since the focus of these assignments is critical thinking). Use your own words and build on the ideas of others. When material is copied verbatim from external sources, it must be enclosed in quotes.

• A reference page which follows APA requirements should be included.
• The essay should be organized in a clear and coherent manner.
• The paper should be double spaced with font size of 12.

The well-being of students is not only reflected in their classwork but also the kind of health services offered to them by the institution. The institution has a program for regular check-ups on pre-diabetes Mellitus and signs of hypertension, which could result in chronic renal disease. An assessment was done to determine how effective the program is, and some of the measures that can be implemented to make it more helpful. This paper will focus on a communication plan for the results of evaluating the program.
The evaluation's results will be shared through an email. Such a medium of communication will make it easy for the evaluation team to make any necessary follow-ups. As well, it will be easy to respond to the feedback and, where appropriate, establish a discussion. The evaluation aims to come up with ideas on areas where the program has not been effective or how it has been helpful to students. Therefore, the communication plan will allow for the responsible team to discuss the possible solutions to the identified loopholes. Besides, communicating through emails will enable the team to copy the message to any other involved parties (Milani, 2019).
The report will be sent to student health services and student affairs services. The body has student leaders, elected by the students to ensure that everything in the college runs smoothly. Therefore, if changes will be necessary to the program, then it is this body that will push the administration into effecting such changes (Astin, 2012).
