Errors in Process
Physical errors in a design are usually brought about by flaws that have taken place in a process. This flaw might be in the planning, examination, production, repair, or some other combination of predetermined actions (Reason, 2000). Some of the most common errors are errors or omission and errors of commission. In the first omission, an individual fails to do what is expected of them while errors of commission lead people to do what they are not expected to do (Hollnagel, 2011). Errors can incorporate wrong calculations, illogical hypothesis, wrong interpretation, or even a refusal to follow the laid down procedures. The process of operating a system needs to be conducted meticulously just as in designing the system itself.
Summary of the Chernobyl Disaster
The 1986 Chernobyl nuclear power plant crisis in Ukraine was caused by design confines together with process flaws in the course of the test. On the specific day that the disaster took place, a total of four Reactor Bolshoy Moshchnosty Kanalny (RBMK) were fully operational at the location of the Chernobyl. The past Soviet Union had chosen the RBMK reactor plan since it could produce electricity for domestic use as, well as produce plutonium that was needed in the manufacturing of nuclear weapons (Munipov, 1992).
The designers of the RBMK failed to include a containment shell that could bar radioactive substances from spreading into the atmosphere in the event of an accident. The Chernobyl facility had been scheduled for a routine on April 26, 1986. This particular experiment was meant to establish whether the kinetic energy produced by the revolving turbines would be enough during a power outage to aid the cooling pumps in the facility for the one minute that was needed until the standby generators took over this role. For this particular test, the engineers decided to put off the coolant system. During the operation, only ten reactors were in operation down from the mandatory 30. This meant that any slight rise in temperature would spiral out of control (Munipov, 1992).
As the reactor began losing power, the control rods were taken away from the central point of the nuclear something that aggravated the fission rate. The engineers then tried to return the control rods as a way of arresting the situation. However, the sudden surge in temperature had distorted the channels something that made it impossible to reinsert the reactors in a proper manner.
