Roger's Diffusion of Innovation theory is a particularly good theoretical framework to apply to an EBP project. However, students may also choose to use change models, such as Duck's Change Curve Model or the Transtheoretical Model of Behavioral Change. Other conceptual models presented such as a utilization model (Stetler's model) and EBP models (the Iowa Model and ARCC Model) can also be used as a framework for applying your evidence-based intervention in a health care setting.
In 500-750 words (not including the title page and reference page), apply a change model to the implementation plan.
Apply one of the above models and carry your implementation through each of the stages, phases, or steps identified in the chosen model.
In addition, create a conceptual model of the project. Although you will not be submitting the conceptual model you design in Topic 5 with the narrative, the conceptual model should be placed in the appendices for the final paper.

Behavior change is one of the most effective systems through which individuals can enhance effectiveness in their medication processes. In this section of the study, the chosen model for enhancing the TeleHealth Change is the Transtheoretical Model of Behavioral Change, which a process by which states that, the health behavior change is a process which is embedded in the adoption of six major stages, which include: the pre-contemplation stage, the contemplation stage, the preparation stage, the action stage, the maintenance stage, as well as the and termination (Prochaska&Velicer, 1999). The Transtheoretical Model of Behavioral Change shall be applied in this project to enhance effectiveness in the interpretation of the evidence-based intervention in a healthcare setting, where the major aim shall be the explanation of how the model relates to the telehealth change process (Velicer et al., 1998). The Transtheoretical Model of Behavioral Change model enhances the ability to understand how individuals can improve their health through regular exercise, keeping a keen interest in their diet, and also the elimination of the unhealthy lifestyles, like smoking. In understanding the general operation of the Transtheoretical Model of Behavioral Change, it is important to understand the individuals who are used in the study as the effects they show in their lives also indicates the appropriateness of the model.

The Transtheoretical Model of Behavioral Change has six major stages which are essential to understanding how the model works (Leino, c. 2014). The first stage of the Transtheoretical Model of Behavioral Change is the Pre-contemplation stage, which is usually characterized by the inability of an individual’s inability to acknowledge that there is a problem and that there are various things which need to be changed, and of importance the behavior change (Leer et al., 2008).
