Purpose
The purpose of this assignment is to explore a critical concept in nursing. Patient safety is one of the essential concepts in nursing. Regarding this, patient safety involves preventing patient harm by implementing healthcare delivery systems that prevent errors, learn from errors in the system, and are based on an organizational culture that emphasizes safety. In the article “Aggregate Analysis of Sentinel Events as a Strategic Tool in Safety Management Can Contribute to the Improvement of Healthcare Safety,” Hooker, Etman, Westra, and Van der Kam (2019) explore how a combination of more than one root cause of sentinel events can facilitate the identification of sentinel events to offer insights on how organizations can improve patient safety. The following paper will detail the key findings of the paper and explore how the findings can be used to improve healthcare safety in healthcare organizations.
The findings from the study are based on a retrospective cross-sectional study conducted in a teaching hospital based in the Netherlands. Hooker et al. (2019) clustered the various elements and variables of sentinel events to learn the patterns of failure in the organization of interest using root cause analysis. The data collected revealed 21 sentinel events in the study site that stemmed from 132 root causes, which reveals the multi-factorial nature of the events that threaten patient safety. The reported events tended to occur in operational theaters and wards, mostly in elderly patients or patients who required multi-disciplinary care. In my professional experience, I have experienced the vulnerability of the identified population and can attribute it to the specificity of care type required by the identified groups. In the study, Hooker et al. (2019) found that 76.2 percent of permanent consequences were reported among the events.
