In 500-750 words (not including the title page and reference page), apply a change model to the implementation plan. Include the following:
Rogers' diffusion of innovation theory is a particularly good theoretical framework to apply to an EBP project. However, students may also choose to use change models, such as Duck's change curve model or the transtheoretical model of behavioral change. Other conceptual models presented such as a utilization model (Stetler's model) and EBP models (the Iowa model and ARCC model) can also be used as a framework for applying your evidence-based intervention in clinical practice.
Apply one of the above models and carry your implementation through each of the stages, phases, or steps identified in the chosen model.
In addition, create a conceptual model of the project. Although you will not be submitting the conceptual model you design in Topic 5 with the narrative, the conceptual model should be placed in the appendices for the final paper.
Prepare this assignment according to the APA guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

Rogers’ diffusion of innovation theory is a particularly good theoretical framework to apply to an EBP project. Other conceptual models presented such as a utilization model (Stetler’s model) and EBP models (the lowa model and ARCC model) can also be used as a framework for applying your evidence-based intervention in clinical practice. Lewin’s change model is another model that can be applied in evidence based projects. Lewin’s change model has three major steps; Unfreezing, Change (Transition), and Refreezing stages.
Lewin’s Change Model
Lewin’s Change Model was developed by psychologist Kurt Lewin in 1950s (Lewin, 1951). Lewin (1951) explained the Lewin theory by giving an analogy of shaping a cube of ice to a conical shape. The first stage was to refreeze the ice into liquid form (Unfreezing). The transformed ice can be changed to the desired shape (Change). The new shaped product must then be solidified (Refreezing). The three steps in the analogy are also applied in evidence-based projects including clinical settings as shown in Fig 1 below.

The first step in Lewin’s Change theory is to unfreeze the current situation (Kritsonis, 2005). This is crucial in changing the status quo of an organization. Unfreezing is important in overcoming the resistance forces in individuals and the conformity to an existing group.
