As noted in the Learning Resources, systems theory provides a meaningful and beneficial means of examining challenges in health care organizations. To do this effectively, however, it is essential to assess all system components, as some may be relatively healthy while others are problematic.
For this Assignment, you apply systems theory to the examination of a problem in a department or a unit within a health care organization. (Note: You may use the same problem you identified for the Discussion as long as it meets the criteria for this assignment.)
To prepare:
Review the Meyer article, “Nursing Services Delivery Theory: An Open System Approach,” in this week’s Learning Resources. Focus especially on the information presented in Table 1 (p. 2831) and Figure 2 (p. 2833).
Reflect on your organization or one with which you are familiar. Within a particular department or unit in this organization, identify a problem the staff is encountering.
Using Table 1 in the Meyer article as a guide, analyze the department or unit, identifying inputs, throughput, output, cycles of events, and negative feedback. Consider whether the problem you have selected relates to input, throughput, output, cycles of events, and/or negative feedback.
Think about how you could address the problem: Consider what a desired outcome would be, then formulate related goals and objectives, and translate those goals into policies and procedures.
Research professional standards that are pertinent to your identified problem.
Reflect on the organization’s mission statement and values. In addition, consider how addressing this problem would uphold the mission and values, while improving the organizational culture and climate.

The emergency department (ED) at the Johns Hopkins Hospital has had an issue of overcrowding for quite some time now. The staff in the ED is always overwhelmed with patients who need care, yet the flow of patients out of the ED is limited. The ED input in the organization includes the inflow of patients requiring emergency care. This is in line with what Meyer and O'Brien-Pallas (2010) refer to when describing the concept of input. In addition, the resources and materials put into the ED comprise of inputs. Such resources and materials include nursing staff who form part of the human resources and hospital equipment, respectively. The throughput includes decisions regarding the order of patient treatment as they come into the ED, surgical scheduling, utilization of nursing staff, and access to specialty services. The throughput basically entails all the processes and procedures that the department carries out to ensure that patients are referred to the right care. According to Meyer and O'Brien-Pallas (2010), output refers to the transfer of the final product into the external environment. The ED output thus entails the transfer of incoming patients from the ED and to either inpatient care or back to the community, based on the diagnosis and the care required by each patient coming through the ED. This is how the ED is able to regulate the volume of patients in emergency care. The cycle of events at the ED entails the repetition of activities in the ED such as patient intake, admission or discharge to maintain the flow of patients. The patients’ outcome in the ED affects the hospital’s revenues and accreditation and once the ED has achieved either of this, the cycle starts again. Negative feedback in the ED is experienced when patients reveal dissatisfaction with care and in our organization, this mostly occurs due to overcrowding.
The issue of overcrowding in EDs has become a major issue in a majority of hospitals across the country. According to Phillips, et al. (2017), overcrowding in the emergency department contributes to negative patients’ outcome because of poor patient care, prolonged length of stay in the ED, and high medical errors. Overcrowding in the ED highly relates to input because as mentioned earlier, ED input is concerned with the inflow of patients to the emergency rooms. In this case, a high number of elderly patients as noted by Morley, Unwin, Peterson, Stankovich, and Kinsman (2018), due to the high number of elderly people with chronic diseases in the country, contributes to overcrowding. Another cause of overcrowding is the high number of patients with urgent care needs, which still relates to input in the ED. Further, overcrowding can also be termed as a throughput problem because all the throughput activities in the ED influences the efficiency of registration, boarding and discharge of patients from the ED. As revealed by Morley, et al. (2018), the shortage of nursing is a contributing factor to the inefficiency experienced in the emergency rooms, which consequently leads to overcrowding.
