An important consideration when working with patients is their cultural background. Understanding an individual's culture and personal experiences provides insight into who the person is and where he or she may progress in the future. Culture helps to establish a sense of identity, as well as to set values, behaviors, and purpose for individuals within a society. Culture may also contribute to a divide between specific interpretations of cultural behavior and societal norms. What one culture may deem as appropriate another culture may find inappropriate. As a result, it is important for advanced practice nurses to remain aware of cultural considerations and interpretations of behavior for diagnosis, especially with reference to substance-related disorders. At the same time, PMHNPs must balance their professional and legal responsibilities for assessment and diagnosis with such cultural considerations and interpretations.

For this Assignment, you will practice assessing and diagnosing a patient in a case study who is experiencing a substance-related or addictive disorder. With this and all cases, remember to consider the patient’s cultural background.

Substance-related and addictive disorders are cognitive disorders that affect a person’s behavior and brain, leading to an inability to control substance abuse like cocaine, nicotine, and marijuana. Addicts will continue to abuse the drugs irrespective of the harm caused to their health. The addictive disorder usually starts through experimenting with drug use for certain individuals, and then the drug use becomes more regular (Baird, 2019). The level of addiction differs from one person to another. For instance, a person may quickly get addicted to opioids. An addict might require a higher dosage to get high. Efforts to stop abusing drugs can lead to withdrawal symptoms like strong cravings. Healthcare professionals should know the signs and symptoms of addictive disorders and ways to assess and diagnose them. During the diagnostic interview, the healthcare practitioner can access various assessment tools. This study will use the DSM-5-TR classification of substance-related and addictive disorders to help in assessing the diagnosis of the case study patient.     
Subjective:
CC (chief complaint): Lisa Tremblay is in an F.L. detox facility and opted for a long rehab treatment. She confesses using opiates, cannabis, and alcohol daily.  
HPI: L.T. is a 33-year-old female seeking medical help to control her Hep C+. She discloses abusing opiates valued at $100 daily and marijuana 1 to 2 times weekly on a medical marijuana I.D. card. She has also been abusing alcohol and using vodka daily. She has a history of drug-related arrests. The differential diagnosis can originate from certain symptomology, including a high level of GGT that indicates the liver is not functioning well, ALT could lead to liver damage, and a low level of albumin shows an underlying medical condition, and UDS positive opiates use (Wook Kim, 2020). The symptoms prevalence period is unknown. Early sexual abuse at the age of 6-9 years has left Lisa traumatized, affecting her life with reduced sleep time to 5-6 hours, poor appetite, and preferring to get intoxicated rather than eat.     
Past Psychiatric History: 
* General Statement:
* The patient does not have any record of previous psychiatric treatment or assessment. She is considering long-term rehab and treatment for her Hep C+, which requires a first clean-up. Her father sexually abused her at 6-9 years old, traumatizing her. Lisa admits to using drugs and alcohol, including opiates, cannabis, and vodka. She has also been arrested in the past for drug paraphernalia possession.  
* Caregivers (if applicable): N/A
* Hospitalizations: She has no hospitalization history.
* Medication trials: Lisa has no history of psychotropic medication reported. The patient has a medical card for cannabis that she uses 1-2 times weekly.
* Psychotherapy or Previous Psychiatric Diagnosis: No history of psychiatric diagnosis or psychotherapy.
