Locate a copy of an advanced directive (AD) that complies with the laws of the state in which you work. Complete the advance directive for yourself. Do not turn in your AD with your paper. The AD is for your personal use.
In your paper, Part I:
Identify where you obtained the AD and explain its compliance with state law.
Conduct research (1-2 sources) to learn more about the AD in your state and explain how it works.
Discuss how easy or difficult it was to complete the AD. Your comments should be specific and both objective and subjective. Part I
The Advance Health Care Directive is an important document in health care. I obtained a copy of the advanced directive (AD) online and completed it. The document is available at the State of California – Department of Justice (State of California, 2021). Under California State law, individuals hold the right to express their health care wishes and to make their health decisions, even when they unable to physically do it (State of California, 2021). The state consolidated different previous forms on health care preferences into a single Advance Care Directive.
California encourages individuals to think about their health wishes especially, end-of-life decision. Once an individual makes the decisions to complete an AD, the individual should keep the copy in an accessible location or safe while letting loved ones know about one’s intentions (State of California, 2021). A person can also prepare an estate/financial plan to secure long-term health of one’s assets. Individuals can also choose to plan their hospice or palliative care for their end-of-life decisions. California palliative care ensures that a person can inform one’s physician, family, and friends about one’s health preferences including any special treatment one may want or not want (State of California, 2021). Finally, California advises individuals to designate a person that will carry out the wishes and complete their details in the AD form.
Completing the AD is direct and almost easy. For example, the process involves filling in the details of one’s wishes in case one is incapacitated (Andrews, 2017). Each requirement is clearly described and presented in the form for an individual to easily fill it out. However, the emotive aspect of the AD is difficult. I found it challenging to complete the AD because it touches on complex and serious decisions that affect’s a person’s life (Andrews, 2017). For example, one is supposed to make a decision on whether to continue or not to continue life support when suffering a terminal condition. I found that I was ill equipped making serious decisions about my health because it is something that I have never taken time to think about. The challenge of having to face the likelihood of one’s death or incapacity to make key decisions about one’s health makes it difficult to complete the form.
Part II
Physician Orders for Life-Sustaining Treatment (POLST) form offers seriously ill patients increased control over their medical or end-of-life care (Cedars Sinai, n.d.). POLST allows individuals to make decisions about the treatment choice, extraordinary measures that can be taken such as whether to be put or not to be put on ventilator or feeding tube, and CRP treatment (National POLST, 2021). The form is a bright pink form that is carried or found with the physicians during the patient treatment. POLST can help prevent unwanted or unproductive treatments by giving the patient more control over his or her heath while also helping reduce family suffering.
