Situation
Mr. John Smith a 75-year-old client was recently diagnosed with Parkinson’s Disease and will require a Home Health Aid to assist him in daily activities such as feeding, mobility, exercise, transportation if needed, and reminders to take medications at the right time.
Background
He is 5’9 and weighs 160 lbs. The client denies any use of alcohol, or drugs. He has been diagnosed with Hypertension, Arthritis, and had a stroke 8 years ago. He has fallen five times over the last year due to gait instability. He lives in a one-bedroom apartment on the third floor by himself. He has a daughter in her 30’s that comes to visit twice a week.
Learning Style
Mr. Smith stated that he is an auditory and visual learner when it comes to educating himself. He says he usually listens to podcast and reads magazines with pictures. He states he has no knowledge or interest of using a computer because it undermines learning. He only uses his phone to listen to podcast. The client prefers to read a pamphlet or listen to an educational podcast to learn.
Readiness of the learner
The client is unhappy to hear of his new diagnosis. Although the patient is not pleased with the diagnosis, he is aware that he needs to take action if he wants to slow the progress of the disease. The client understands the difficulty it will take but he states he is willing to work hard. The daughter will be overseeing his activity to make sure he is on the right path.
Goal Development
Firstly, a patient suffering from Parkinson’s disease needs to join support groups so as to interact with other patients and individuals who have had the same diagnosis. This platform will allow them to share experiences, information, aspects of the disease and counselling. Secondly, research has shown and proven that exercise may slow the progression of Parkinson’s disease (Clarke, 2017). Under close evaluation and documentation of the patient’s progress, exercise helps patients stay fit both mentally and physically, improve the art of socialization and the quality of life, making them strong and flexible.
Thirdly, patients suffering from Parkinson’s disease need to be taught some basic safety rules that they oblige. In this case, as the disease progresses, they need to be cautious of the risk of falling and also while they are driving. Fourthly, Parkinson's patients tend to have speech problems; a slurred voice and speaking quietly due to the weakening of the voice box and its effect to the mouth, tongue, lips and throat (Clarke, 2017). Therefore, they will require a speech therapist who will examine their condition so as to prevent issues with swallowing which could increase problems with aspiration, coughing and choking, hence preventing pneumonia.
Learning Outcome
The patient has a better understanding of his diagnosis, Parkinson’s disease. He has better knowledge of the signs and symptoms of Parkinson’s and is able to differentiate if any new signs and symptoms occur.
