Please read this entire document to gain understanding of the project scope and those tasks you will complete.

Task 1:
Choose from one of the following stages: pregnancy,
List the specific nutrition issues affecting individuals in your selected stage.
Task 2:
Choose a disease or medical condition that could possibly, or commonly, affect your individual. This could be anything ranging from diabetes to cirrhosis.
Using the ABCDs of nutritional assessment, create a case study based on an actual client or patient. You can also make one up. Include the following in your case study:
Anthropometrics, relevant biochemical tests, clinical assessment, and dietary intake analysis.
Be sure to include a list of common medications that may be used to treat your patient's condition and identify potential herb/nutrient/drug interactions that may be relevant.
Identify your client's cultural background and give clues as to their socioeconomic status and psychosocial variables. For example, your patient may be a senior living alone on a fixed income or might be living in a nursing facility with reputed staff.

Background
The nutritional care plan focuses on diabetes among pregnant women where alerted nutrition and nursing interventions are necessary to improve nutrition and health outcomes. In addition, knowledge on adequate nutrition is necessary, where an expectant mother is expected to know more about dietary patterns and caloric intake (Alexopoulos, Blair & Peters, 2019). There is also diabetic management and self-monitoring. The ABCDs of nutritional assessment focus on anthropometric, biochemical, laboratory, clinical, and dietary evaluation methods.

Anthropometrics Tests
Anthropometrics focuses on measuring the body, weight, height, and proportions, essential when examining the pregnant mother. Based on this, there is calf circumference, body mass index (BMI), waist-to-hip ratio (WHR), and waist circumference (WC). The measurements are compared with standards considering the age and pregnancy stage while considering the nutritional status. There is also an evaluation of whether the mother is under or nutrition. At the pre-pregnancy weight of 165 pounds and height at 5 feet 7, the BMI was 25.8, slightly above the overweight level of 25-29.9. During the pregnancy, she gained 40 pounds and weighed 205 pounds with a BMI of 32.1
Biochemical tests
Biochemical assessment focus on checking a person’s level of nutrients in their urine, blood, or stools, and lab tests are necessary to evaluate the nutritional status.
Glycemic tests focus on glycosylated hemoglobin (A1C) and oral glucose tolerance tests (Berger, Gagnon & Sermer, 2016). HbA1c (glycosylated, or glycated hemoglobin) levels indicate the blood glucose levels over six to eight weeks. The glucose tolerance test measures the blood sugar levels before and after drinking solution with glucose and measures high glucose levels where 200 mg/dL and higher indicates diabetes (CDC, n.d.). Drinking the solution with glucose is recommended in pregnant women between the 24th and 28th weeks if there are suspected of having gestational diabetes.
* The nutritional anemia profile is another test, and pregnant women are at high risk of iron deficiency and adverse health outcomes. Iron deficiencies are tested, focusing on the hemoglobin levels, B12, iron, and folate.
* Blood pressure where the target is below 130
* Macronutrient and micronutrient screening, including vitamin D, help to evaluate whether there is nutrient deficiency.
* Kidney function tests include creatinine clearance.
* Thyroid function tests
