Much like assessing or recommending pharmacotherapeutics for other conditions or disorders, as an advanced practice nurse, you may encounter a patient who presents with a psychological disorder. Understanding the guiding principles related to treating patients with psychological disorders as well as the effects of pharmacotherapeutics on a patient’s overall health and well-being is critical for the safe and effective delivery of care.
This week, you examine types of drugs prescribed to patients with psychological disorders. You also examine potential impacts of pharmacotherapeutics used to treat psychological disorders on a patient’s pathophysiology.

Discussion: Decision Making When Treating Psychological Disorders
Psychological disorders, such as depression, bipolar, and anxiety disorders can present several complications for patients of all ages. These disorders affect patients physically and emotionally, potentially impacting judgment, school and/or job performance, and relationships with family and friends. Since these disorders have many drastic effects on patients’ lives, it is important for advanced practice nurses to effectively manage patient care. With patient factors and medical history in mind, it is the advanced practice nurse’s responsibility to ensure the safe and effective diagnosis, treatment, and education of patients with psychological disorders.

Pharmacotherapy in Major Depressive Disorder
There are three primary treatment regimens for depression, namely, pharmacotherapy, depression-specific psychotherapy, and somatic therapy. Drug therapy or psychotherapy is proven effective for mild to moderate depression, while the combination of the two is the best treatment for severe depression (Rosenthal & Burchum, 2018).
Drug Selection
Insomnia is a sleep disorder characterized by the difficulty in inducing sleep or staying asleep. This can be acute, which lasts from one night to a few weeks, or chronic, wherein the difficulty happens at least thrice a week for at least three consecutive months (Morin, Drake, Harvey, et al., 2015). In the case given, the patient has had insomnia for approximately six months. Thus, an antidepressant that is suitable for the patient is the atypical antidepressant, Mirtazapine (Rosenthal & Burchum, 2018).
It has also been stated in the case that the patient is suffering from poor concentration, which hinders him from working effectively and weight gain for the past few months (Rosenthal & Burchum, 2018).
Atypical Antidepressant: Mirtazapine
Mirtazapine is an atypical antidepressant with a generic name of Remeron. It belongs to the first-line treatment regimens for major depressive disorder (Rosenthal & Burchum, 2018).
Its mechanism of action is to block the presynaptic a2-adrenergic receptors. Thus, inhibiting adrenergic effects and promoting neurotransmitter release, namely 5-hydroxytryptamine (5-HT2 and 5-HT3) and norepinephrine. Furthermore, this has been found to inhibit histamine release by blocking histamine receptors. Hence, promoting sedation and weight gain (Rosenthal & Burchum, 2018)....
