Bipolar spectrum disorders can be screened using the Mood Disorder Questionnaire (MDQ). The Mood Disorder Questionnaire is one of the best screening instruments to evaluate Kristin’s condition (MDQ). A nurse, or other competent health professionals can quickly and easily complete the instrument, a one-page paper, and pencil self-report inventory (Dumont et al., 2020). It consists of thirteen yes-or-no questions created using the DSM-5 criteria and clinical expertise. The tool also includes a further question that asks if some of the reported hypomanic or manic symptoms coincided (Boland & Verduin, 2022). A patient is stated to have bipolar I disorder if they correctly answer seven or more of the following questions. To evaluate Kristin and provide an appropriate diagnosis, it would be beneficial to give her a nine.
2- Provide a rationale for the screening tool you selected.
It will only take Kristin five minutes to complete the Mood Disorder Questionnaire assessment test. MDQ looks at certain bipolar disorder-related behaviors. Additionally, functional impairment and symptom co-occurrence are assessed. Despite not being a diagnostic tool, the MDQ guides comprehensive clinical assessment. Evidence suggests that MDQ aids in choosing the best drug to recommend to a patient. In addition, the instrument is utilized to track a patient's response to therapy (Liu et al., 2021).
3- Determine the most likely diagnosis for Kristin based on the available information.
Kristin has displayed most of the symptoms linked to bipolar I disease. She expresses dissatisfaction with herself (worthlessness), feels irrationally enraged, has trouble sleeping and staying asleep, and has lost weight. A patient can be identified as having bipolar I disorder if they have experienced at least one episode of hypomania or mania, per the DSM-5 criteria (Miller & Black, 2020). Additionally, bipolar I illness is linked to at least one manic episode, which may co-occur with significant depressive or hypomanic episodes (Boland & Verduin, 2022).

5- Provide a treatment plan with rationale for each plan step.
Due to interactions with other drugs, medication reconciliation must be performed before prescribing any medication, including herbal or vitamin supplements. A mood stabilizer is one of the drugs that will be considered. Abilify as a monotherapy, the beginning dose will be 15 mg, administered once daily (Cunha, 2022). Depending on the clinical outcomes, the dose may be raised to 30 mg if she doesn't get better. Prozac (Fluoxetine) 20 mg PO daily is an additional treatment option for Kristin's illness. The FDA has authorized fluoxetine to treat the major depressive disorder (Cunha, 2022). If the original prescription is unsuccessful, the dosage will be raised to 40 mg daily. A follow-up should be conducted after two weeks to evaluate the efficacy of the treatment strategy.
