Throughout the duration of this course, you will build a strategic plan for a healthcare affiliated program (e.g., Telemedicine Services for Cancer Patients in Rural South Carolina) of your choice. You will build upon the strategic plan throughout the SLP assignments in this course.
For your Module 1 SLP, identify a healthcare-affiliated program that you are interested in researching and developing a strategic plan for. Your topical area should be specific and cater to a particular population and/or area. In 2–3 pages, complete the following in Part 1 of your strategic plan:
Title Page: Identify the title of your strategic plan. (This section is not to be included in the 2–3 page count).
Topic Relevance: In 1–2 paragraphs, explain your topic selection and its importance to the identified population and/or area.
Mission and Vision Statements: The mission and vision statements explain what your plan is trying to achieve. Your mission and vision statements should each be 1 or 2 sentences followed by a paragraph (for each statement) of explanation. The health program for inner-city individuals with Type 2 diabetes aims to improve care access and empower patients to take an active role in managing their condition. Campbell and Egede (2020) assert that in many cases, individuals from low-income neighborhoods face challenges managing their diabetes compared to those from wealthier communities. Campbell and Egede (2020) note that minority groups such as African Americans, Hispanics, and Native Americans living in inner cities face barriers such as lack of knowledge, limited social support, and poor self-management support. Living in the inner-city intensifies the disease burden because of other factors such as poverty, violence, discrimination, poor education opportunities, and unemployment. Research indicates that inner-city dwellers are less likely to access recommended health care, which increases poor health outcomes (Campbell & Egede, 2020). The difficulties faced by individuals from poor neighborhoods limit access to essential information in managing Type 2 diabetes.
Additionally, individuals from inner cities display limited health literacy. Han et al. (2019) claim that lower health literacy increases when education opportunities are limited, especially in poor neighborhoods. Health literacy among patients with Type 2 diabetes is linked to better health outcomes (Han et al., 2019). Among patients with diabetes, poor health literacy can increase the risk of complications such as retinopathy, worsening blood sugar levels, and lower levels of self-management (Han et al., 2019). In this case, the health program for inner-city patients with Type 2 diabetes offers a chance to improve access to necessary health information. Individuals gain an opportunity for better education so that they adopt self-management practices.

A program that educates inner-city patients with Type 2 diabetes provides essential information on managing the disease. In many cases, diabetes complications occur when diabetes is not managed effectively (Campbell & Egede, 2020). Inner-city individuals face many barriers that discourage proper access to education and health literacy. The program aims to ensure that such obstacles can be overcome and opportunities identified to ensure improved health outcomes. The health program would also empower patients with Type 2 diabetes to realize the power they have to take care of their health.
