49-year-old patient with rheumatoid arthritis comes into the clinic with a chief complaint of a fever. Patient’s current medications include atorvastatin 40 mg at night, methotrexate 10 mg po every Friday morning and prednisone 5 mg po qam. He states that he has had a fever up to 101 degrees F for about a week and admits to chills and sweats. He says he has had more fatigue than usual and reports some chest pain associated with coughing. He admits to having occasional episodes of hemoptysis. He works as a grain inspector at a large farm cooperative. After extensive work-up, the patient was diagnosed with Invasive aspergillosis

A 49 years old patient is presented with fever (reaching 101o F) as the chief complaint accompanied by chills, sweats, and occasional pain that comes with coughing. While the patient medical history indicates a diagnosis of rheumatoid arthritis and hemoptysis and reliance on atorvastatin and methotrexate, the current diagnosis indicates the presence of invasive aspergillosis. Therefore, the purpose of the current paper is to explain the presenting symptoms for the assigned scenario, identify the genes associated with the development of the disease, and explain the immunosuppression process and its effect on the body.
An Explanation for Symptoms and Signs
The critical symptoms presented by the patient are fever (for weeks), excessive fatigue, chest pain that comes alongside coughing, chills, and sweats. These symptoms are consistent with the CDC's list of invasive aspergilloses (IA) of the lungs’ symptoms. These include fever, chest pain, coughing with blood (the hemoptysis condition present in the patient), sweat & chills (resulting from the fever), and fatigue (CDC, 2020). According to Challa (2018), invasive aspergillosis occurs in immunosuppressed individuals. In this case, the patient already has rheumatoid arthritis, an autoimmune disease. Autoimmune diseases are conditions in which the immune system mistakenly attacks the body. The lung is the most common organ involved in IA. The extent to which the disease manifests depends on the degree of immunosuppression.
Identification of Genes Associated with Disease Development
Some genes in the human body can exacerbate the risk of the development of IA. According to Lupianez et al. (2020), polymorphisms with the ARNT2 and CX3CR1 genes are associated with increased risks of developing this condition. Thus, individuals are more vulnerable to the disease of the genes are coupled with conditions like the weakened immune system and long-term immunomodulatory therapy. Further, Cunha et al. (2013) argue that TLR4 polymorphism has been linked to chronic IA in immunocompetent individuals. Thus, aside from the already mentioned risk factors, genes are also responsible for the condition's development.
