Evolving Practice of Nursing and Patient Care Delivery Models

Essay Instructions:

The RN to BSN program at Grand Canyon University meets the requirements for clinical competencies as defined by CCNE and AACN using nontraditional experiences for practicing nurses. These experiences come in the form of direct and indirect care experiences in which licensed nursing students engage in learning within the context of their hospital organization, their specific care discipline, and their local communities.
As the country focuses on the restructuring of the U.S. health care delivery system, nurses will continue to play an important role. It is expected that more and more nursing jobs will become available out in the community, and fewer will be available in acute care hospitals.
Write an informal presentation (500-700 words) to educate nurses about how the practice of nursing is expected to grow and change. Include the concepts of continuity or continuum of care, accountable care organizations (ACO), medical homes, and nurse-managed health clinics.
Share your presentation with nurse colleagues on your unit or department and ask them to offer their impressions of the anticipated changes to health care delivery and the new role of nurses in hospital settings, communities, clinics, and medical homes.
In 800-1,000 words summarize the feedback shared by three nurse colleagues and discuss whether their impressions are consistent with what you have researched about health reform.
A minimum of three scholarly references are required for this assignment.
While APA format is not required for the body of this assignment, solid academic writing is expected and in-text citations and references should be presented using APA documentation guidelines, which can be found in the APA Style Guide, located in the Student Success Center.

Title: Evolving Practice of Nursing and Patient Care Delivery Models
Summary of Nurse Colleague’s Response
My nurse colleagues had differing views and opinions regarding the future of nursing in the United States. However, most of their views supported my research findings. Owing to the restructuring of the US healthcare system, and the emergence of new primary care models, my colleagues felt that indeed the new models were to play a critical role in the delivery of cost effective primary care services across the care continuum. However, there was consensus that NMHCs were to play an even more important role compared to medical homes and accountable organizations. There was concurrence that these new healthcare delivery models would spark demand for nurses in the community rather than in the acute care hospitals(Auerbach et al., 2013). Regarding the study findings that suggested an acute shortage of primary care providers driven by an increase in patient demand, their opinions were varied. Tracy stated that the demand for practitioner nurses was going to increase, even though the increase would be insufficient to counter the supply. On the other hand, Greg and Maurine concurred that increased demand for primary care will overwhelm the traditional model, causing patients to seek for cost effective alternatives that promise continuity. Although they agreed that Affordable Care Act had a significant impact on the emerging primary care delivery models, they felt that transformation of delivery care models towards patient centered models would create more job opportunities for nurses and nurse practitioners rather than physicians (Auerbach et al., 2013).
Greg was of the opinion that increased demand for primary care will shift the demand away from the traditional physician-centered model towards more comprehensive, integrated, quality driven, patient-centered models. He was of the opinion that this scenario will be largely triggered by cost effectiveness and primary care service quality. This observation was in agreement with my research findings. Tracy and Maurine observed that due to the new models’ potential to offer primary care to both insured and uninsured populations, they were likely to witness extremely high demand that would ultimately surpass supply. However, they said, this would depend on continued state funding without which these models would become inconsequential. Maurine stated that, “..In my view, state funding is the lifeblood that shall drive NMHC’s, medical homes and Accountable Organisations towards achieving their intended objectives”. Her suggestion was in agreement with my research findings. Indeed state funding will be critical both on the demand and supply side of the healthcare delivery model (O’Leary et al., 2014).There was concurrence that Affordable Care Act had triggered demand, and the state was obligated to implement policies that would stimulate a corresponding growth in supply to meet this demand.Tracy observed that a paradigm shift in healthcare delivery towards patient centered models is the way forward as it ensures greater continuity of ...
