The following review will establish the effectiveness of the individualized discharge plans so that my management can realize why they need to shift from routine discharge plans to individualized discharge plans. Importance of the Evidence-Based Solution Healthcare facilities ought to develop individualized discharge plans because it offers many benefits. Supporting the continuity of health care between the community and the healthcare setting is one of the benefits of the individualized discharge plans (Gonçalves‐Bradley et al.,2016). In addition, the individualized discharge planning will lessen the initial hospital length of stay as well as lessen the readmission rates.
