“Amiya Nidhi was a young woman in her twenties who had recently immigrated to the United States from India. She was in the hospital to give birth. Her support person was her sister, Marala. Marala kept telling her to get an epidural, but Amiya said that even though she would like one, she could not get one; her husband would not allow it. Cindy, her nurse, overheard the conversation. Having learned that husbands are the authority figure in the traditional Indian household, she went to speak with Mr. Nidhi. She explained why an epidural would be advisable. She said that he seemed pleased that she came to him about it. He said he would think about it, and let her know.”

Cultural competence is essential in health practitioners, especially those dealing with clients from diverse cultures. In some cultures, the father is the overall decision-maker, even on health and education. For instance, in the United States, the parents make health decisions for their children up to 18 years, after which the post-teenagers can make their decisions (Sandoval, 2019). From the scenario, it is evident that the husband/fathers make healthcare decisions for the family, and their spouses oblige. Also, Nidhi cannot decide to receive the injection without her husband's approval because she believes that husbands and fathers are the family's overall decision-makers.
The four C's involved in the scenario are critical thinking, communication, collaboration, and creativity. The medical practitioner exhibited crucial consideration in determining the cultural barrier that prohibited Nidhi from making individual health decisions (Park & Yoo, 2019). The nurse applied critical thinking skills, identified the problem, and decided to consult Nidhis's husband. The medical practitioner then communicates with the husband and gives him the advantages of the injection during childbirth. Through creativity, the nurse achieved her desires without disrupting the authority structure of her clients. Through collaboration with the patients' family, the healthcare providers, Cindy and Nidhis's husband, managed to offer treatment solutions in a culturally competent way.
In the given scenario, I would change how the medical practitioner approaches the concept by first getting the injection's patients' perspective.
