Health History
Mr. M., a 70-year-old male, has been living at the assisted living facility where you work. He has no know allergies. He is a nonsmoker and does not use alcohol. Limited physical activity related to difficulty ambulating and unsteady gait. Medical history includes hypertension controlled with ACE inhibitors, hypercholesterolemia, status post appendectomy, and tibial fracture status postsurgical repair with no obvious signs of complications. Current medications include Lisinopril 20mg daily, Lipitor 40mg daily, Ambien 10mg PRN, Xanax 0.5 mg PRN, and ibuprofen 400mg PRN.
Case Scenario
Over the past 2 months, Mr. M. seems to be deteriorating quickly. He is having trouble recalling the names of his family members, remembering his room number, and even repeating what he has just read. He is becoming agitated and aggressive quickly. He appears to be afraid and fearful when he gets aggressive. He has been found wandering at night and will frequently become lost, needing help to get back to his room. Mr. M has become dependent with many ADLs, whereas a few months ago he was fully able to dress, bathe, and feed himself. The assisted living facility is concerned with his rapid decline and has decided to order testing.

When it comes to medical examination and diagnosis, there must be a thorough understanding of the pathological manifestations of a disease and how it is treated or managed, keeping in mind its progression and how it affects the patient over the long stretch. In the case of Mr. M, his memory lapses are a clear indicator of suffering from Alzheimer's. By failing to remember the names of his family members and wandering off and forgetting his room, there is a serious chance that he is rapidly deteriorating in terms of his mental health. There is also some chance that he has dementia. His increased agitation and the fact that he is dependent on outside help to carry out his routine activities cause concern. According to Budson and Solomon (2015), increased fear and anxiety during agitation are common symptoms of Alzheimer's. Dementia is always a case when it comes to age, and in this case, the aging Mr. M, at 70 years, might be having a worsening instance of the same. His BMI is 28.4 since his weight is 87 kg and his height 69.5 inches. This is indicative of obesity.
Primary and Secondary Diagnosis
A primary diagnosis that can be made at first is urinary tract infection. Considering that there are leucocytes in his urine, it is therefore quite probable that his urinary tract has got some infection. The most common infections usually occur in either the bladder or the kidney. This is especially so given the fact that he is taking Lipitor and Lisinopril currently. According to Marks (2020), these two medications usually cause a breakdown in muscle tissue, which results in kidney failure. Any issues with the kidney also have a resultant impact on the liver. It is also important to look at his blood pressure. At 123/78, it is clear that he has high blood pressure since this is above the standard of 120. His blood pressure readings are a pointer to prehypertension, whose range is usually between 120/80 and 139/89. It is usually a warning that the patient can quickly develop hypertension, which is a serious risk for stroke (Beckerman, 2021). Luckily enough for Mr. M, it has been detected early enough and can therefore be managed accordingly.
Having pointed out Alzheimer's like the most likely ailment, Mr. M must undergo all the medical tests on his memory to ascertain that.
