Using this MAP-IT approach, a step-by-step, structured plan can be devised to tailor one’s community needs.
The first step in building a healthier community is to mobilize key individuals and organizations to form a community-wide coalition. Most communities already have health departments and other governmental agencies that are responsible for public health services. Many communities also have coalitions of key individuals and organizations that are organized to address specific issues, for example, block associations or neighborhood watch groups. These groups often represent diverse interests and resources for addressing issues that are vital to building and maintaining the health and stability of the entire community. A coalition will often, of course, work with the health department and other health organizations in the community. However, it can also help mobilize a wider range of other resources to address health issues.

Problem Overview
As established in the previous paper, the Millers have a myriad of health issues that require both immediate and long-term interventions. In the family, both parents have a history of substance abuse, are obese, and smoke. Both parents are hypertensive. The 40 years old mother has developed diabetes while the father has liver complications. Their 14 years old son is asthmatic while the 18 years old daughter has a history of substance abuse and is reported to have undergone two abortions. The family is undereducated with the mother having a BA in Literature, and the father did not study past high school. It is a low-income family with minimal insurance.
Problem Scope
A 2017 National Survey on Drug Use and Health found that 1 in 35 children (or a total of 7.5 million) lived in a household with at least one parent who had had illicit drug use in the past year. The study further found that 1 in every eight children aged 17 or younger lived in households with at least one parent having substance use disorder (Lipari & Van Horn, 2017). Studies have linked substance abuse with poverty. Low-income families, like the Millers, are more likely to struggle with substance abuse because poverty increases addiction risk factors such as stress, low self-esteem, feeling of hopelessness, lack of social support, and poor access to care (Staff, 2018). In helping the Millers; therefore, the plan will include efforts to elevate the family from poverty and substance abuse.Further, obesity, diabetes, and hypertension are key aspects that will need the attention of the coalition. CDC reports that between 2017 and 2018, the prevalence of obesity was 42.4%, having increased from 30.5% (in 1999), with over 70 million Americans suffering from the condition (CDC, 2019). Obesity may lead to other complications such as hypertension and diabetes, further burdening the minimum insurance of such families. CDC further reports that nearly half of adults in the United States (108 million) have hypertension. According to a 2017 report by the American College of Cardiology and American Heart Association (CDC, 2018). Respiratory chronic diseases such as asthma are also prevalent in American communities according to the Asthma surveillance data by the CDC.
Thus, the problems faced by families such as the Millers are socioeconomic and, therefore, require the intervention of the community. The mission of the coalition is to ensure a decrease in household substance abuse, elevation from poverty, and immediate care and management of current medical issues in such families. Thus, the Millers require both immediate and long-term interventions as part of the plan of action by the coalition.
