GA wade wrote, “We’ve built our medical system around the long tail”. This happened after his experience in offering medication to one of his patient who was suffering from cancer. His efforts in carrying out surgery and chemotherapy helped the patient live a little longer but it did not prevent him from dying. Gawade uses this statement to refer to health services that have created expensive edifice meant to dispense medical equivalent of that which prepares patient to the next level of pain and death (Gawade, A. 2014). He suggests that terminal illness patient should be allowed to die peacefully at home instead of waiting for a miraculous recovery in hospital bed which gives hope where there is no hope.
