This assignment requires you to interview one person and requires an analysis of your interview experience.
Part I: Interview
Select a patient, a family member, or a friend to interview. Be sure to focus on the interviewee's experience as a patient, regardless of whom you choose to interview.
Review The Joint Commission resource which provides some guidelines for creating spiritual assessment tools for evaluating the spiritual needs of patients. Using this resource and any other guidelines/examples that you can find, create your own tool for assessing the spiritual needs of patients.
Create a survey to assess the subject's spiritual need during the interview. The spiritual needs assessment survey needs a minimum of five questions that can be answered during the interview. During the interview, document the interviewee's responses.
Submit the transcript of the interview. The transcript should include the questions asked and the answers provided. Be sure record the responses during the interview by taking detailed notes. Omit specific names and other personal information from the interview.
Part II: Analysis
Write a 250-500 word analysis of your interview experience. Be sure to exclude specific names and other personal information from the interview. Instead provide demographics such as sex, age, ethnicity, and religion. Include the following in your response:
1. What went well?
2. What would you do differently in the future?
3. Were there any barriers or challenges that inhibited your ability to complete the assessment tool? How would you address these in the future or change your assessment to better address these challenges?
4. Describe the spiritual experience you had with your patient, family member, or friend using this tool. How does this tool allow you to better meet the needs of your patient?
5. Did you discover that illness and stress amplified the spiritual concern and needs of your interviewee? Explain your answer with examples.
Submit both the transcript of the interview and the analysis of your results.

Religion and spirituality are two important aspects in the lives of many people. They are a powerful force that greatly influences the well-being of people and help them in dealing with the challenges of life. The healthcare professionals acknowledge this fact and consider spiritual assessment a part of providing holistic care to patients. However, some healthcare professionals overlook the role played by religion and spirituality in patient care. The Joint Commission requires the inclusion of spiritual assessment in healthcare organizations so that they can make an effort in seeking spiritual impressions and insight about patients and their problems and look into the best way to assist them (Hodge, 2006). Using the requirements and guidelines provided by the Joint Commission and other sources I came up with my own tool for assessing the spiritual needs of patients that consists of five questions.
The Interview
James is 34 years old, Caucasian, father to three, husband and son. The interview with James took place at his home in Ithaca, New York. James works as a mechanic at a local repair shop and has lived in Ithaca for the last fifteen years. He is married and has three children, two sons, and a daughter. James was a perfect candidate for the interview due to his experiences in dealing with a medical condition. He was diagnosed with type 2 diabetes five years ago.
“Hello, thank you for accepting this interview. The course am taking requires me to conduct an interview on spiritual needs assessment that may be of use in the future in my services as a nurse. I appreciate that you spared your time for the interview. It is okay if you prefer not to answer any of the questions and feel free to stop the interview anytime, if necessary.

1. What is your religion or how could you describe your spiritual beliefs?
My religion is Christianity. I consider myself a man of God, me and my family we are devoted Christians, I give my tithe, attend the service every Sunday and weekdays when I have a chance and do good things in my life and to those around me.
2. Why is religion important in your life?
Religion is an important part of my life. God is the giver of life and all that is in this world, I owe him my life and strength. I believe that I have a good relationship with God and that gives me peace and strength to carry on despite the challenges of life.

I am a very active member of our local church. I have the support of my friends and the church regarding my faith and health.
