Chronic illnesses have become one of the most prevalent healthcare problems in the United States of America (U.S.). According to Donald et al. (2013), chronic illnesses are responsible for about seven in every ten deaths in the U.S. In this sense, several groups across the country have recognized h the significant problems with chronic disease. They have resulted in the establishment of sustainable, replicable programs that work to improve health and lower costs among persons with chronic conditions. The initiatives/programs seek to address various gaps concerning providing care to individuals with chronic illnesses.
One of the initiatives to help improve the quality of health among individuals with chronic illnesses is the Evercare program. It is an initiative that provides several healthcare services to persons with long-term or advanced disease, advanced age, and disabilities (Smith, 2003). Over the years, Evercare has grown to become among the most extensive health care coordination programs. The Evercare program's main objectives are to ensure improvement in managing specialized acute care and restructuring of service delivery for the frail populations and those with chronic illnesses. As a result, one of the specific goals of the initiative is to ensure that they are available physicians and nurse practitioners available to provide healthcare services to the persons long-term or advanced illness, advanced age, and/or disabilities (Smith, 2003). The initiative is designed to mainly depend on electronic medical records to ensure they are facilitated coordination between the multiple healthcare providers.
