Understanding family structure and style is essential to patient and family care. Conducting a family interview and needs assessment gathers information to identify strengths, as well as potential barriers to health. This information ultimately helps develop family-centered strategies for support and guidance.
This family health assessment is a two-part assignment. The information you gather in this initial assignment will be utilized for the second assignment in Topic 3.
Develop an interview questionnaire to be used in a family-focused functional assessment. The questionnaire must include three open-ended, family-focused questions to assess functional health patterns for each of the following:
Values/Health Perception
Nutrition
Sleep/Rest
Elimination
Activity/Exercise
Cognitive
Sensory-Perception
Self-Perception
Role Relationship
Sexuality
Coping
Select a family, other than your own, and seek permission from the family to conduct an interview. Utilize the interview questions complied in your interview questionnaire to conduct a family-focused functional assessment. Document the responses as you conduct the interview.
Upon completion of the interview, write a 750-1,000-word paper. Analyze your assessment findings. Submit your questionnaire as an appendix with your assignment.
Include the following in your paper:
Describe the family structure. Include individuals and any relevant attributes defining the family composition, race/ethnicity, social class, spirituality, and environment.
Summarize the overall health behaviors of the family. Describe the current health of the family.
Based on your findings, describe at least two of the functional health pattern strengths noted in the findings. Discuss three areas in which health problems or barriers to health were identified.
Describe how family systems theory can be applied to solicit changes in family members that, in turn, initiate positive changes to the overall family functions over time.
Cite at least three peer-reviewed or scholarly sources to complete this assignment. Sources should be published within the last 5 years and appropriate for the assignment criteria.
Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center.

Family-Focused Functional Assessment
Family Structure
The family that was interviewed for this family-focused functional assessment are the Smiths. They are an Asian-American family of four. The couple is both 55 years old, the husband is American while the wife is Chinese. They have a 25-year old daughter and a 20-year old son. The father works in an office with a 9 am to 5 pm schedule. The mother owns her own online business with a more flexible time. Their daughter works as a work-from-home programmer 5 days a week with varying hours, while their son is a student taking culinary arts. They are devout Christians living in a middle-class neighborhood.
Health Perception
Health perception is defined as a person’s assessment of their health which is important to know because it affects motivation and other health behaviors (Leite et al, 2019). Mr. Smith was a former athlete so he values exercise and healthy living even now. At work, Mr. Smith is required to undergo annual checkups that include ECG, X-ray, 2D-, blood tests, etc. Mrs. Smith has been diagnosed with diabetes for 5 years and is taking oral medications as prescribed by her endocrinologist which she visits twice a year. Their children were raised to be health-conscious with a mixture of standard medicine, Christianity, and traditional Asian medicine. The family drinks alcoholic beverages occasionally and does not smoke cigarettes, except for the son.
Nutrition
The Smiths are conscious about their diet, especially since their son entered culinary school and was educated with proper nutrition and diet. Now, the family is conscious of nutrition labels when going to the grocery stores. The family’s diet consists of mostly protein, vegetables, and fruits because they are consciously limiting their carbohydrate intake to support Mrs. Smith in managing her diabetes. But, she admits to having a hard time controlling her sweet tooth.
Sleep/Rest
The Smiths aim to develop a healthy sleep routine by going to bed at around 10 pm and getting at least 7 hours of sleep per night. Mrs. Smith reports that sometimes she is disturbed by the need to urinate. The daughter also reports feeling tired often even upon waking up.
Elimination
Mr. Smith noticed that he started going to the restroom to defecate, around 2 to 3 times a day after his gallbladder was removed due to gallstones, 10 years ago but his physician said that this was normal. Mrs. Smith reports frequent urinating at night, even used the term “nocturia” because she already had this checked and her endocrinologist said it was a part of diabetes.
Activity/Exercise
The family tries to be physically active and have access to a treadmill and weights at home. Mr. Smith and his son are fond of exercising regularly, at least 3 times a week as a form of bonding, while the ladies try to do cardio at least once a week.
