I am meeting with a neighbor for the practicum experience. He is greater than 65 years of age with peripheral neuropathy. He has no comorbid. Typically this occurs in diabetics, but in his case he is debilitated with pain and numbness and nerve pain in feet. No medical history other than GI problems unrelated . In this assessment, you’ll assess the patient, family, or population health problem that will be the focus of your capstone project. Plan to spend approximately 2 hours working with a patient, family, or group of your choice to explore the problem from a leadership, collaboration, communication, change management, and policy perspective. During this time, you may also choose to consult with subject matter and industry experts about the problem (for example, directors of quality or patient safety, nurse managers/directors, physicians, and epidemiologists).
To prepare for the assessment, complete the following:
Identify the patient, family, or group you want to work with during your practicum The patient you select can be a friend or a family member. You’ll work with this patient, family, or group throughout your capstone project, focusing on a specific health care problem.
Begin surveying the scholarly and professional literature to establish your evidence and research base, inform your assessment, and meet scholarly expectations for supporting evidence.

Peripheral neuropathy (PN) entails a set of neuropathic symptoms that result from abnormalities in the function and structure of peripheral motor, sensory and autonomic neurons (Ogle et al., 2020). The disease leads to reduced lower extremity sensation that can cause significant morbidity (Hicks et al., 2021). While the condition mainly affects adults with diabetes, it also affects people with normal glucose tolerance (Hicks et al., 2021). In the neighborhood, a 68-year-old male has peripheral neuropathy with no comorbidities. A 2-hour meeting with the neighbor and his sons revealed that he is debilitated with numbness and nerve pain in his feet. They reported that their father had no other medical condition related to PN but only suffered from gastrointestinal problems. A nursing manager was present at the meeting to offer nursing insights on PN.
Hicks et al. (2021) reported that the general weighted prevalence of peripheral neuropathy in adults with diabetes was 13.5%, while that for adults without diabetes was 11.6%. Ogle et al. (2020) reported that PN is high in people above 55 years, with a prevalence of 8%, while it occurs in the general population with 2.4% prevalence. PN is relevant and significant to older patients since it causes mortality (Hicks et al., 2021). Anastasi and Klug (2021) explained that PN increases the likelihood of mortality, impairs quality of life, and leads to significant morbidities. PN is relevant to nursing practice for baccalaureate-prepared nurses since they gain practical knowledge and skills for managing the condition.
A median follow-up of 13 years in the U.S. population conducted by Hicks et al. (2021) reflected that 2140 deaths occurred due to PN. It revealed that PN was associated with mortality in participants with long-lived diabetes. PN was also significantly linked to mortality in patients without diabetes (Hicks et al., 2021). It causes lower limb amputations, work disability, and functional impairments (Hicks et al., 2021). Anastasi and Klug (2021) explained that PN leads to loss of sensation, ulcerations, chronic infections, falls, amputations, foot injury, tissue damage, and poor wound healing.
Various nursing actions are required to prevent and manage PN. Since PN affects adults with or without diabetes, nurses can offer practical advice to patients at risk or those with PN to prevent or manage high blood sugar. Anastasi and Klug (2021) explained that nurses educate patients on modifying risk factors such as hypertension, obesity, and hyperglycemia through exercise and glycemic control.
