Guidance and coaching is a core competency of advanced practice nursing. How do you think guidance and coaching in the advanced practice role is different from the RN role of teaching/coaching? Are there certain elements of this competency that are more important than others? How does the teaching and coaching role fit with a wellness versus sickness model of care?
Guidance is directing, advising and counseling patients, and it is closely related to coaching, but less comprehensive and while nurses offer guidance, they empower the patients to manage the care needs through coaching. In the RN role, teaching and coaching is associated with the transfer of information and skill as well as patient education (Hamric et al., 2014). There is the transfer of information and skill where there is assistance to choose outcomes, while there is also patient engagement, patient empowerment. The APN also raises awareness while also implementing and sustaining behavior change. There is emphasis on a smooth transition from one phase, status or condition to another in advanced practice roles.
The guidance and coaching competency are important, while coaching deals with facilitating patient engagement and empowerment, guidance encourages information transfer where the patients are expected to make a decision. Both Guidance and Coaching support high levels of patient engagement compared to teaching which requires a medium level of engagement (Hamric et al., 2014). Coaching and guidance require knowledge expertise, and ability to use evidence-based interventions to address patient and family needs while utilizing adult teaching strategies (Cooke, Gemmill, & Grant, 2008).
Wellness care promotes health through regular care without having to wait for symptoms or complications, compared to the sickness model of care where there is emphasis on treating and addressing the sickness. Provider-patient relationships are necessary in promoting health outcomes and quality of life. Teaching and coaching fit in with the wellness care model.
