David, an 11-year-old boy in fifth grade, was brought for evaluation by his parents who expressed concerns over his anxious behavior regarding developing Acquired Immunodeficiency Syndrome (AIDS). David was a well-adjusted boy and who was doing well, but suddenly began to exhibit extremely disruptive behaviors related to his fears of AIDS. This began about four to five months ago. He also began to express concerns about getting sick, to wash his hands frequently, to seek reassurance continually, and to demonstrate rituals and avoidance.
David expressed concern many times that he can get AIDS from looking at a stranger with AIDS. Despite his parents' reassurance, David insisted on washing or bathing for approximately one hour each time he gets home after being outside. He also began to have difficulty focusing on schoolwork. When reading assigned materials for school, David frequently experienced severe anxiety over possibly missing a word or misunderstanding a sentence, and he would reread the material many times. Eventually, he was not able to complete assignments and his grades began to deteriorate.

Anorexia Nervosa
It is a chronic eating disorder characterized by significant weight reduction, more than 15% of the person’s average body weight, secondary to extreme diet restrictions, leading to adverse nutritional consequences detrimental to the patient’s health (Katzung, 2017). Some of its complications include retarded pubertal growth and development, dehydration, cardiovascular diseases (i.e., mitral valve prolapse, cardiac arrhythmias, postural tachycardia, hypotension), electrolyte imbalances (i.e., hypokalemia, hypomagnesemia, hypocalcemia), gastrointestinal changes (i.e., delayed gastric emptying, constipation, pancreatitis), and endocrinologic and reproductive problems (i.e., protracted amenorrhea, hypothalamic-pituitary-gonadal axis dysfunction) (Herpertz-Dahlmann & Dahmen, 2019; Substance Abuse and Mental Health Services Administration, 2016). The disease peaks in adolescence, but it can be present during childhood. However, it is not easy to analyze data because children and adolescents are often reported as combined information. Typically, adolescent females are affected. However, some studies suggest that boys can be more affected than girls, depending on the region (Herpertz-Dahlmann & Dahmen, 2019).
Pharmacological Treatment
Antipsychotics
These block the dopaminergic receptors in the brain, and the atypical antipsychotics have additional serotonergic blocking ability (Aigner et al., 2019). Milano et al. (2013) revealed that although these do not directly impact weight gain. Nevertheless, these are successful in enhancing body image, reducing fear of weight gain, and hampering agitation, pseudohallucination, and obsessive-compulsive symptoms. Second-generation antipsychotics, based on evidence, are more successful in decreasing the symptoms.
Antidepressants
Specifically, selective serotonin reuptake inhibitors (SSRIs) diminish the effect of serotonin that is the key in appetite, impulse control, and mood dysregulation (Aigner et al., 2011).
