A 38 year old homeless woman comes into the emergency room with complaints of weight loss, lethargy, fever, and productive cough. She lost her home when she started drinking heavily and was fired from her job for missing work and drinking on the job. She lives in a homeless encampment downtown because she does not like the rules the shelter has. She lives in a tent with 2 other women and the area is crowded. She is able to grab a meal at a neighborhood church twice a week, but spends any money she gets on beer.
The emergency room physician suspects that the patient has tuberculosis.

The risk factors for getting a TB infection include; alcohol and drug abuse and several diseases like cancer, HIV and diabetes. The factors that are presented in the patient is alcohol abuse that was so bad that she was fired from her job and lost her home. There are three test that can be used to diagnose a patient for TB. These tests include chest X-ray, tuberculin skin and blood tests. The chest X-ray are done to check for any changes in the lungs that may indicate TB signs or previous TB scars (Lehne, 2013). The tuberculin skin test is done by the injection of a solution that contains a protein, which is composed tuberculosis bacteria in the skin. However, the test is not conclusive because it can show a false positive if a person was previously received the Bacilli Calmette Guerin vaccine.
