  • Review this week's Learning Resources and reflect on the insights they provide.
  • Go to the Stahl Online website and examine the case study you were assigned.
  • Take the pretest for the case study.
  • Review the patient intake documentation, psychiatric history, patient file, medication history, etc. As you progress through each section, formulate a list of questions that you might ask the patient if he or she were in your office.

  • Identify people in the patient’s life you would need to speak to or get feedback from to further assess the patient’s situation. Include specific questions you might ask these people and why.
  • Explain what physical exams and diagnostic tests would be appropriate for the patient and how the results would be used.
  • List three differential diagnoses for the patient. Identify the one that you think is most likely and explain why.
  • List two pharmacologic agents and their dosing that would be appropriate for the patient’s ADHD therapy based on pharmacokinetics and pharmacodynamics. From a mechanism of action perspective, provide a rationale for why you might choose one agent over the other.
  • If your assigned case includes “check points” (i.e., follow-up data at week 4, 8, 12, etc.), indicate any therapeutic changes that you might make based on the data provided.
  • Explain “lessons learned” from this case study, including how you might apply this case to your own practice when providing care to patients with similar clinical presentations.

For the young 26 year old mother and her 8 year old daughter with Attention Deficit Hyperactivity Disorder (ADHD), there is a need to evaluate whether there are other family members with similar conditions. The patient has been diagnosed with ADHD, Generalized Anxiety Disorder (GAD) and stress, and treatment that results in the remission of the symptoms will improve daily functioning. Methylphenidate is one of the most commonly used medications to control the ADHD symptoms and some of the common brands are Concerta and Ritalin (Stahl, 2017). Methylphenidate works by increasing norepinephrine mostly through dopamine actions by blocking the reuptake (Stahl, 2017). Methylphenidate and amphetamine are stimulants that are combined with other stimulants and when deciding on the most appropriate medication considering the changes in attention span and concentration and sedation is necessary to improve outcomes.
What do you believe are the pressing issues that need to be resolved to improve your health outcomes? The case for this is that there is prioritization of the health intervention and the goals of treatment, where the effectiveness of interventions is evaluated over time and goal management treatment is potentially useful (In de Braek et al., 2017). The success of the interventions as reflected by observable outcomes after medication helps to improve the health outcomes since there are modifications based on the patient’s progress. Medication can be increased, maintained, reduced or changed to improve health outcomes.
Is there anyone else in your family who has been diagnosed who ADHD? The risk of ADHD increases if one or more family members have the condition, mostly where parents and the children tend to be diagnosed with ADHD. Researchers have focused on the influence of genetics and heredity in explaining the family risk factors in ADHD (Brikell, Kuja‐Halkola & Larsson, 2015). Children who are diagnosed with ADHD the risk of problems associated with the condition are likely to persist into adulthood and considering family history is necessary to undertake effective intervention. The family continues to have great value in itself and will continue to play important roles in people's lives, as a social institution.
