Background -Case Study: An Elderly Iranian Man with Alzheimer’s disease
Mr. Akkad who is 76 year old and Iranian was accompanied by his eldest son who expressed concern about his father’s recent behavior including disinhibited behaviors, strange thoughts and failing to attend religious activities unlike before. This Mini-Mental State Examination (MMSE) was used and the patient was diagnosed with major neurocognitive disorder due to Alzheimer’s disease (presumptive).
Decision Point One-
Decision choice : Begin Exelon (rivastigmine) 1.5 mg orally BID with an increase to 3 mg orally BID in 2 weeks The other decisions are “Begin Aricept (donepezil) 5 mg orally at BEDTIME “ and “Begin Razadyne (galantamine) 4 mg orally BID”.
Beginning Exelon (rivastigmine) 1.5 mg orally was chosen as the preferred choice since it is well tolerated and FDA approved for Alzheimer’s disease. The pseudoirreversible medication works by inhibiting acetylcholinesterase (AChE) without inhibition of butyrylcholinesterase (BuChE) (Stahl, 2014). Aricept (donepezil) was not chosen as it has transient gastrointestinal side effects and disturbances, which would affect the patient’s quality of life. Razadyne (galantamine) increases acetylcholine, and I did not choose the option as it is may result in gastrointestinal (GI) side effects, nausea and dizziness, which would affect the client’s quality of life (Stahl, 2014).
I hoped to reduce behavioral disturbances, improve the MMSE score and reduce the severity of his symptoms that he finds pleasure and joy in activities that he previously enjoyed such as religious service and activities. The patient does not attend religious services/activities, and continues to be disinhibited, he has memory confabulation and the MMSE again scores is 18 out of 30. The medication takes time to take effect and there was increase in dosage in decision point two.
In the scenario when considering client’s pharmacokinetic and pharmacodynamic processes, his advanced age and behavior are considered as they affect how the intervention is carried out with there be slower absorption and metabolism of the medications for the elderly.

Decision choice: Increase Exelon to 4.5 mg orally BID
The two choices were: “increase Exelon to 6 mg orally BID” and “Discontinue Exelon and begin Namenda (memantine) 10 mg orally BID”.
